Trosolwg o'r swydd

We are seeking a highly motivated and experienced Community Diabetes Specialist Nurse to join our multidisciplinary team, delivering high-quality diabetes care across community settings. This is an exciting opportunity to play a key role in improving patient outcomes, promoting self-management, and reducing health inequalities for people living with diabetes.

The post holder will contribute to the Kingston Tier 3 Diabetes team by: -

  • Delivering evidence-based practice for patients with Type 2 Diabetes in the community with Multidisciplinary Team clinics across the borough of Kingston.
  • Working collaboratively with the Kingston Tier 3 Diabetes Multidisciplinary Team and others to meet the needs of patients and carers.
  • Supporting and providing mentorship to other team members and Primary Care colleagues.

  • Deliver high-quality nursing care to patients with diabetes in community settings, under the guidance of senior clinicians.
  • Undertake clinical assessments and contribute to the development and implementation of individualised care plans.
  • Support the management of patients with Type 2 diabetes, including blood glucose monitoring and lifestyle advice.
  • Assist with and, where trained, initiate and adjust diabetes treatments (including insulin) in line with agreed protocols and competency level.
  • Provide structured and informal education to patients, carers, and families to support self-management.
  • Work as part of the multidisciplinary team, liaising with GPs, dietitians, podiatrists, and secondary care services.
  • Offer advice and basic diabetes education to primary care staff and other community healthcare professionals.
  • Recognise and escalate concerns relating to complex cases, deteriorating patients, or diabetes-related complications.
  • Support hospital admission avoidance by monitoring patients in the community and identifying early signs of deterioration.
  • Maintain accurate, timely clinical records in accordance with Trust policies and NMC standards.
  • Participate in audit, service evaluation, and quality improvement activities.
  • Contribute to health promotion initiatives, including diabetes prevention and early intervention strategies.
  • Maintain and develop own clinical knowledge and competencies in diabetes care through ongoing professional development.
