Our vision is for the people of Croydon to experience well-co-ordinated care and support in the most appropriate setting, which is truly person-centred and helps them to maintain their independence.
We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through the use of technology, ensuring patients have the safest best care.
This post created with the goal of enhancing our current Front Door Frailty services and developing our Hospital at Home pathways. As such the successful candidate would be working between our acute and community sites. They would be working closely with both the well-established Acute Care of the Elderly in-patient team, our newly appointed frailty assessors and our Rapid Response community team.
Our innovative acute frailty service has been running for approximately 6 years with a 16 bedded acute frailty unit (ACE) and a rapid access GP “hot clinic”. There are plans to extend this further via establishing a Frailty “Same Day Emergency Care” unit with the aim of proactively pulling frail elderly patients from ED and the acute take to undergo Comprehensive Geriatric Assessment. Additionally, the Frailty SDEC would take direct referrals from London Ambulance Service, thus avoiding the need for ED attendance altogether.

Croydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon. The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust. Around 4,100 staff provide services for a population of over 360,000 people who are relatively young with a high level of ethnic diversity.
Our main hospital site, Croydon University Hospital is one of the busiest in London. We’re also leading the way in providing more healthcare outside hospitals at clinics, specialist centres and in people’s homes.
