An exciting opportunity has arisen for an experienced and motivated Community Children’s Nurse to join our Community Children’s Nursing Service on a one-year fixed-term contract to cover maternity leave.

As an integrated organisation, we are uniquely positioned to provide seamless care across hospital and community settings. Our service continues to evolve in line with the NHS ambition to deliver more care closer to home through neighbourhood working, proactive case management and a strong focus on optimising health and wellbeing. This role offers the opportunity to play a key part in that transformation and contribute to shaping the future of community children’s services.

 You will provide expert clinical care and case management for children and young people aged 0–19 years with acute, chronic and complex health needs, supporting them to remain safely at home wherever possible and reducing avoidable hospital admissions through coordinated, proactive care planning. We are seeking an individual who is passionate about delivering high-quality, child-centred care in the community and who is committed to proactively optimising health outcomes for children and young people. In return, you will have the opportunity to further develop your leadership skills within a supportive senior leadership team and be part of a service that is actively redefining community healthcare delivery.

Job Summary

  • To be effective in organising and planning the day to day management of the Children’s Hospital at Home Team caseload, ensuring the individual needs of children and their families are met.
  • Have an open minded approach to work and demonstrate ability to multitask in order to get the job done through flexible hours and locations
  • To identify and manage risks to patients and staff.
  • To be responsible for line management, education and training, and support and mentoring of colleagues.
  • To work collaboratively and develop effective relationships within the multi-disciplinary team and other departments and organisations
  • To work with the Clinical Team Leader (CTL) to develop innovative clinical practice and represent the team on their behalf when needed.
  • To promote the work of the Children’s Hospital at Home, to ensure a quality service is delivered

The post holder will work in accordance with the Department of Health, Nursing and Midwifery Council and trust policies relating to infection control, safeguarding children/adults, standards of record keeping, medicines management, learning disabilities, privacy and dignity and other policies as appropriate to the designated areas. Be responsive to the changing needs within the Directorate in relation to local and national policy and guidance.
