Triage assessment Clinical Nurse Specialists - Band 7

We are looking for an enthusiastic nurse to join our friendly and supportive Macmillan Cancer Nursing and Support Team to provide high quality care for patients referred to our USC colorectal pathway. The post holder will work with the Macmillan Colorectal Nurse Consultant, Triage Clinical nurse specialists, Cancer Nurse Specialists, Colorectal surgeons and Gastroenterologists, and multiprofessional team to provide a straight to test USC pathway.

You will need to have excellent nursing assessment and communication skills as the post holder is required to undertake telephone assessments with patients. You will need to be confident in following clinical protocols and making autonomous decisions. The post holder will also be expected to independently review results and manage the patient’s onward referral as appropriate. Our Macmillan Nursing and Support Team have a strong track record of providing and supporting professional development. The post holder will be supported to contribute to the leadership, education, and development of nursing and clinical teams across the Trust.

Prif ddyletswyddau'r swydd

To be responsible for developing and delivering the patient  USC pathway including specialist nursing advice for colleagues and patients.To deliver evidence based, clinical diagnostic protocols, including “straight to test”.To use highly advanced communication skills in supporting patients throughout the diagnostic pathway; using advanced clinical assessment skills to ensure all relevant clinical information is available to the multi-disciplinary team.To use specialist nursing knowledge to review and interpret all clinical investigatory results and determine next pathway stage; including discharge and referral back to GP, or referral to specialist services as required.To demonstrate a higher level of practice through the provision of specialist nursing knowledge and expertise in the delivery of diagnostic pathways, including LGI suspected cancer.

To be self-directing and work autonomously, identifying and maintaining supportive networks; but escalating as appropriate for supervision of complex caseloads.

To have responsibility for management for junior nurse colleagues working within the TAS, adhering to Trust HR processes and staff development; and allocation of duties within the nursing team.

To support the Trust in achieving compliance with Urgent Suspected Cancer (USC) and Referral to Treatment (RTT) waiting time targets.

  • Knowledge and understanding of colorectal cancer pathways including 2ww and 18ww routes
  • • Knowledge of NHS operational and strategic issues and their impact on Colorectal services. • Understanding and knowledge of common gastroenterology anSound knowledge and understanding of diagnostic pathways for suspected Colorectal Cancer including Endoscopy procedures and bowel preparation procedures
