Band 5 Community Staff Nurse – 0-19 Service

Are you passionate about making a difference in the lives of children, young people, and families? We're looking for a dedicated and compassionate Band 5 Community Staff Nurse to join our dynamic 0-19 team.

As part of the wider community nursing and primary healthcare team, you’ll work under the guidance of the Specialist Community Public Health Nurse (Team Lead), delivering a range of services that include health surveillance, health promotion, immunisations, and providing support and advice to families with children and young people aged 0-19.

You’ll collaborate closely with both statutory and voluntary partner agencies to help achieve the best possible health outcomes for children and families. Using evidence-based practice, you’ll contribute to delivering a safe, effective, and high-quality service aligned with professional standards.

In this key role, you will support the implementation of the 0-19 Healthy Child Programme within a progressive universal model—tailoring interventions based on assessed needs and ensuring seamless partnership working across agencies.

Prif ddyletswyddau'r swydd

To support the delivery of the 0-19 Healthy Child Programme (HCP) in providing an innovative and high quality service that helps children, young people, and families to thrive, working without direct supervision to support clients.

The post holder will be responsible for their designated workload within their speciality area with access to the support, advice and management from a Specialist Community Public Health Nurse (SCPHN) clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and  knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge. 

o   Assess, plan, implement and evaluate individual, family focused care, under the delegation of the Specialist Community Public Health Nurse (SCPHN) (Health Visitor/School Health Nurse) and working in partnership with other agencies.

o   Establish and maintain partnerships with families and other agencies in a multi-agency and integrated working environment.

To attend and participate in locality meetings.

o Ensures that a professional service and image is maintained at all time, and acting as a role model forstaff
