Job overview

The post holder will support residential care home staff to positively manage and meet the health needs of individuals with a Learning Disability within the Croydon Area.

The post holder will work collaboratively to ensure people living in care homes have access to enhanced primary care and to specialist services and maintain their independence as far as possible by reducing, delaying or preventing the need for additional health and social care services

Main duties of the job

·         To identify and address health inequalities and ensure that long-term health conditions are identified and addressed 
·         To assess, advise and contribute to meeting the health needs of people with complex physical health
·         Introduce a Training Analysis of residential care home staff to identify areas of further development and educative support.
·         Promote and support up-take of Annual Health Checks with GP Practices/PCN networks
·         Review quality of Health Action Plans/Health Care Passports – Advise/support/facilitate identified health needs 

·         Support training and education 

·         Liaise with GP services to formulate and implement Urgent Care Planning in line with current guidance. Advocate for residents.

·         To work with people who engage in behaviors that challenge (e.g. physical aggression to others, self-injurious behaviors etc.) 

·         Support staff with MCA/BI decision making with GP services/involved health professionals and families.

·         Support care homes hospital admission and complex discharge planning.

·         Ensure that care home staff feel supported and included as part of the MDT and integrated professional network

·         Provide regular reports for LeDeR via the LAC and steering group.

·         To contribute to multi-disciplinary planning and meetings

Person specification

EDUCATION AND QUALIFICATION

Essential criteria
  • RNLD or Degree in Registered Nurse Learning Disability
Desirable criteria
  • Evidence of post qualification training experience up to MA level in a relevant field
  • Evidence of further associated qualifications/CPD in Learning Disability Practice.

KNOWLEDGE AND UNDERSTANDING

Essential criteria
  • Knowledge of current research and legislation in the field of Learning Disability Nursing and ability to translate evidence into practice
Desirable criteria
  • Knowledge of NHS Long Term Plan Ageing Well Project

Experience

Essential criteria
  • 2 years post-registration experience of learning disability nursing
  • Experience of nursing in the community/a Community Learning Disability Team
  • Experience of working with people with learning disabilities with complex physical health needs.
  • Sound knowledge of the principles of clinical governance, national policy and good practice guidance relating to people with learning disabilities. In particular, evidence based practice, clinical audit and the current access to healthcare agenda.
Desirable criteria
  • Experience of working with people with a Learning Disability in an acute/hospital setting.
  • Experience of working with people with learning disabilities who engage in behaviours that challenge.
  • Experience of assessing people with suspected dementia.
  • Experience of assessing people with suspected autism.
  • Experience of CHC framework and implementation
