Living Independently for Everyone

A+E Liaison Occupational Therapist and Physiotherapist  Band 6 (2 posts available)

Post holders will be expected to work weekends/evenings and bank holidays and split shifts to cover busy times of the service

Our vision is for the people of Croydon to experience well-co-ordinated care and support in the most appropriate setting, which is truly person-centred and helps them to maintain their independence.

As demand rises with increasing health needs and advances in clinical care, we are responding to the challenges whilst continuing to deliver the highest quality of care. We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through advanced holistic assessment and ensuring patients have the safest best care that is delivered in their home environment.

Living Independently for Everyone- LIFE
Croydon Health Services are in a new Alliance Partnership with other health and social care providers in Croydon and the LIFE service is one of our new integrated service models. This means bringing together health and social care teams who work to support people to remain independent and healthy in particular to prevent admissions to hospital and shorten hospital stays.

The A+E Liaison is an admission avoidance service available 7 days a week.

Are you a dynamic, flexible, innovative and motivated experienced Occupational Therapist or Physiotherapist with excellent assessment skills? Clinical expertise and good communication skills are required as well as excellent time management and prioritization skills.

The A+E Occupational Therapist  will provide assessment of patients within the A+E department, A+E observation ward and occasionally to patients in their own home during a crisis if required. . This post will have particular emphasis on avoidance of hospital admissions.

The candidates will need excellent knowledge of all community services and will need to work closely with the Multi-disciplinary team in helping to facilitate safe discharges. The candidate will need to be motivated and innovative with strong leadership and communication skills, to support the development of this service.
