Job overview
An exciting opportunity has come up to work as a Band 5 physiotherapist in community neuro-rehabilitation in Ealing.
Staff will benefit from an opportunity to develop knowledge and skills from senior clinicians and the wider Multi-disciplinary team. Our service is committed to staff development by providing regular supervisions, and CPD opportunities.
This role requires a valid HCPC Physiotherapy registration. Candidate must have undergraduate experience in core clinical areas to include neurology and should have trained or worked in the NHS.
Main duties of the job
· To provide an evidence-based assessment, treatment and advisory service to clients with neurological conditions both progressive and acquired such as stroke, traumatic brain injury, Parkinson’s disease, MS, spinal cord pathologies, MND.
· To work flexibly with all other members of the ENABLE team to provide an integrated rehabilitation service to clients in accordance with the philosophy and operational policies of the team
· To see clients in community outpatient clinics or other settings such as their own homes, or leisure facilities
· To manage a varied caseload comprising clients with a range of neurological diagnosis causing neurological impairments that often include musculoskeletal, orthopaedic, cognitive and communication impairments as well.
· To prevent hospital admission and facilitate discharge from, for example, local HASUs discharging early supported discharge patients following stroke
· To participate in departmental continuous professional development, education and training, audit and research projects.
· To manage a clinical caseload using evidence-based clinical reasoning, critical thinking, reflection and analysis
· To use a client centred, goal-orientated rehabilitation approach.
· To participate in departmental clinical supervision that is responsive and personalised e.g. can take the form of formal training, clinical reasoning, peer review, case reviews and reflective practise.
Working for our organisation
Community-based health care in Ealing is delivered in partnership with West London NHS Trust, and Central and North West London NHS Trust, (CNWL) the Hillingdon Hospitals NHS Trust and some other smaller providers, including third sector. Together they have formed a partnership to provide integrated community health services in Ealing. The total value of the new 10 year contract is about £50m per year.
We have also committed to invest in improved links with local voluntary sector organisations (for example ECIL, Ealing Dementia Concern, Ealing Mind) and other community assets for innovation and flexibility.
Detailed job description and main responsibilities
- To undertake a comprehensive assessment of patients including those with diverse presentations.
- To use clinical reasoning skills and thorough assessment on which to base physiotherapy input.
- Formulate and deliver an individual physiotherapy treatment programme based on evidence based practice and treatment options using clinical assessment, reasoning skills and a broad knowledge and understanding of neurological rehabilitation.
- To accept responsibility for a designated caseload of patients, and to organise this effectively and efficiently with regard to clinical priorities and use of time.
- To carry out assessment and treatments of a range of neurological conditions with moderate physical effort on a daily basis.
- To have basic knowledge of neurological pathologies, presentations and management of neurological conditions
- To complete home visits independently or with other team members to provide physiotherapy assessment and input.
- Where required to carry out assessment and treatment of patients referred for group classes in the community sites and out patient clinics.
- To travel independently through the borough of Ealing between patient homes and clinical sites.
- To clarify and explain the patient’s condition to them to enable the formulation of realistic agreed treatment goals and plans. This may sometimes involve the breaking of unwelcome news to the patient and/or their carers.
- To deal sensitively with patients who have high levels of anxiety and depression as part of their neurological presentation.
- To deal sensitively with the relatives and carers of patients.
- To comply with the Trust’s Manual Handling policy and local therapeutic handling guidance at all times.
- To be responsible for maintaining accurate and comprehensive patient treatment records in line with Trust procedures and Chartered Society of Physiotherapy (CSP) standards of practice.
- To write concise, informative reports detailing treatment outcomes and levels of disability.
- To communicate effectively with patients and their parents, guardians or carers, to maximise recovery potential, and to ensure understanding of both conditions and treatment. Communication skills of persuasion, motivation and explanation to encourage patients to undertake their treatment programme.
- To ensure that your own actions support equality, diversity and rights of our patients and their families.
- To assess patient understanding of treatment proposals, gain valid consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
- To maintain accurate record of patients’ attendances and treatments using Systmone.

- Degree Physiotherapy
- State registration with HCPC
- Relevant Post Registration or under graduate courses or in-house training
Desirable criteria
- Membership and/or involvement in National special interest group
Knowledge & Experience
Essential criteria
- Broad under graduate experience in core clinical areas: MSK/ Respiratory/ Community Therapy/ Neuro/ Care of Elderly.
- A good working knowledge of the NHS system of care. (Trained or worked in the NHS)
- Experience of working as part of a team
- Understanding of common neurological diagnoses and the impact of these conditions on client function.
- Understanding the principles of multi-disciplinary teamwork
- Demonstrates an understanding of holistic rehabilitation of clients with complex needs and disability
- Has an understanding of the political and economic situation in relation to health and social care.

- Participation in project activity or audit

- Able to complete a physiotherapy assessment for clients with neuro rehabilitation needs, formulate treatment goals and management plan.

- Problem solving skills to meet client rehabilitation needs.
