Job overview

To lead a team of clinical pathway navigators and adminsitrators and be responsible
for the smooth running of the patient journey from referral to discharge from Trust care
by ensuring that coordinated and streamlined administrative processes revolve
around the patient and their individual needs.
The post holder will also be expected to carry out duties as per discussions with the
Service Manager, Assistant Operations Manager and Operations Manager. The post
is a challenging job in a supported and structured environment

Main duties of the job

This involves developing strong working relationships and working closely with
navigators, clinical pathway administrators and clinical team PAs within their team and
other managers, all key members of the multi-disciplinary team within primary and
secondary care and service users relating to the service specialty they cover to ensure
all patients receive a consistent and effective service. This includes working with
consultants, medical, nursing and therapy teams, divisional and operational
managers, central appointment office, ward administrators, diagnostic and imaging
departments, external agencies e.g. GPs, other hospitals, social services, police etc
