A small team of “System Convenors” has already been established at Hackney Council, working to build networks and partnerships in the borough between key organisations who support the social determinants of health. We are now piloting a Homerton Hospital-based role who will work alongside the existing System Convenor team.
Main duties of the job
The role will report directly to Annabelle Burns, Head of Integration, who will act as the senior project sponsor and will provide support and supervision for the post-holder. The postholder will identify and improve connections, referral pathways and processes between hospital-based services and the voluntary and community sector for the benefit of our patients. They will gather data, insight and share learning and promote the advantages of collaboration and preventative, social determinants-focussed work across Homerton and beyond. The role would suit someone that has established local knowledge and networks and is looking for a new challenge and opportunity to work on a time-limited and impactful project.

Detailed job description and main responsibilities
Service development and delivery
· Establish networks, online systems and methods of communication that enable those currently providing support for the social determinants of health at the Homerton Hospital to form a strong network
· Facilitate shared learning and support amongst the established network
· To develop clear and compelling communication channels, internally and across wider partners
· Identify and improve connections, referral pathways and processes between hospital-based services and the Voluntary and Community Sector, Council services, health and other anchor institutions
· Support the development of the ecosystem of support for residents through relationship building between partners and networking
· To deal with complaints sensitively, avoiding escalation where possible, in accordance with Trust policy
· To plan, participate in and coordinate events across the place-based partnership
Project Management
· Establish and maintain partnership relationships and communicate effectively with a range of key stakeholders
· Lead on this programme of work using initiative, applying reason and managing individual projects, putting forward new and innovative change ideas
· Collate and analyse relevant information/service data, trends, user consultation and feedback,
· Collect and collate research about effective services nationally and internationally to ensure that development and planning are based on good practice and evidence
· Develop project plans including timetabling key milestones, events and decision making points
· Knowledge of performance management and quality improvement evaluation methods
· Act as lead for the project - providing expertise and oversight in the development of the project and enlisting specialist expertise and support as required
- Gather data and insight and share learning in order to help to improve services for marginalised communities and those who often fall through the net of support (but may more often be touching hospital services)
· Prepare and produce concise and insightful communications for dissemination to stakeholders, including drafting and presenting papers and reports for presentation to meetings and committees as required, identifying recommendations and action plans as appropriate
- Promoting the advantages of collaboration and preventative or social-determinants focussed work to colleagues at the Homerton, partners and stakeholders.
· Maintain a risk register for projects and devise strategies for mitigation where possible, escalating through agreed routes as appropriate
· Ensure a continuous quality improvement approach is embedded within the project and governance arrangements, ensuring co-design with residents and staff at all points in the improvement cycle
· Chair, lead and facilitate a range of meetings, networks or other events to support the project delivery
· Ensure projects align with Trust values and is delivered according to relevant Trust policies
· Identify any lessons learned during the project implementation and collate future recommendations
· Ensure effective staff communication and involvement, ensuring staff have the opportunity to be involved in and influence the planning and development of service
Human Resources and Budget Management
· Manage allocated project budget: budget planning, managing ordering process, approving orders, liaising with procurement etc as needed
· Offer training to colleagues across the organisation in group or one-to-one settings as appropriate
· Offer support, supervision and mentoring to other project managers across the organisation as required
Information management
· To maintain high quality record keeping standards in accordance with professional guidelines, service protocols and Trust policies.
Professional
· To demonstrate commitment to training where needed and their own continuous professional development
· To be accountable for their own professional practice and to seek advice and supervision from line manager/supervisor where needed.
· To maintain registration with any relevant registration body (e.g. HCPC)
· To take responsibility for undertaking all mandatory training required by the Trust within the expected time frame.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
