Job overview
The Transfer of Care Hub (ToCH) is based at Homerton Hospital, based within the Integrated Independence Team( IIT) and facilitates discharges across City and Hackney as well as surrounding areas. It is staffed by Homerton Healthcare staff and London Borough of Hackney (LBH). The purpose of the job role is to support the Transfer of Care Hub with Administration duties, being an integral part of the Hub.

Main duties of the job
Together with the team of Clinical Co-ordinators, manage patient information referred into the ToCH.
To provide first point of contact assistance/advice for referrers, health professionals and service users
Liaising with primary, secondary, health and social care providers, to facilitate service users into appropriate discharge pathway.
Assess client’s referral discharge information, ensuring any inappropriate referral is rerouted to the correct pathway or missing information is gathered
To update all electronic systems in conjunction with ToCH Clinical Co-ordinators including referral email account, spreadsheets and Rio/EPR systems.
To assist with the production and analysis of data to produce reports supporting relevant KPIs and targets
To work with other teams, departments or local authority to facilitate timely, appropriate and seamless discharge from hospital into the chosen discharge setting.
Promote independence for service users, enabling them to live in the least restrictive environment whilst ensuring they receive the necessary care and treatment

Initially process all new referrals, evaluate and decide on appropriate pathways, gaining any missing information.
Ensure all referrals required for discharge have been acted upon and will be in situ in time for discharge.
Ensure that all incoming cases are dealt with efficiently and that service user’s needs are assessed in line with statutory timescales
To maintain patient records in accordance with local and national information governance requirements
Identify and report issues surrounding safeguarding adults to line manager, complying with the organisation’s framework
Facilitate and attend multi-disciplinary meetings and/or case conferences to provide relevant information as part of the case-management process
To maintain an up to date working knowledge of legislation, statutory frameworks and codes of practice relevant to the work of the department. This knowledge to be applied to ensure the work of the department is carried out in accord with required practice

Written
Essential criteria
- Good standard of education including GCSE English & Maths or equivalent
- Evidence of continuing professional development
- A competent level of IT skills including user knowledge of core Microsoft Office packages, ECDL or equivalent
Desirable criteria
- NVQ Level 3 (or higher) in relevant domain such as: • Health and Social Care • Administration
- Experience of working with people with medical and/or social issues.
