We have an exciting opportunity for a Band 4 Diabetes Education and Self-Management for Ongoing and Newly Diagnosed (DESMOND) Educator to join our friendly and supportive team within Brent and Harrow Community Diabetes Service.

This is a key role, focusing on delivering DESMOND structured education and supporting patients to better manage their diabetes. The overall aim is to help patients improve their health and quality of life through effective education, coordination, and clinical support.

Main Duties and Responsibilities will be to Deliver DESMOND structured education sessions to patients. Support the administrative aspects of delivering the course. Engage in audit and data collection to support our Primary Care Network. Participate in training and development to become a proficient educator.  Work collaboratively with colleagues across Brent and Harrow to ensure high-quality service delivery.

Requirements: undertake DESMOND training, Strong commitment to always maintaining patient confidentiality, Flexibility to work across sites in Brent and Harrow. Availability for 6-day working (Monday–Saturday), with an expectation of one Saturday in every four weeks.

Main duties of the job

• Commitment to undertaking Desmond Training

• To always ensure patient confidentiality

• Will be required to work cross-site in Brent and Harrow to deliver Desmond.

• 6-day working (Monday – Saturday) – with an expectation of one Saturday in every four weeks.

To follow the prescribed training programme for DESMOND Educators (non-HCP), which will include:

• Observing a local DESMOND education course. Attending 3 separate days of training organised by the DESMOND National Programme which may involve travel to attend.

• Committing to a mentorship programme delivered locally by an assigned mentor (an experienced and accredited healthcare professional DESMOND Educator).

• Using the development workbook and other resources to support ongoing development as a DESMOND Educator.

• To participate in the DESMOND Training and Quality Development Framework.

• Delivery of Desmond Structured Education

• To deliver the DESMOND Newly Diagnosed & Foundation modules together with an accredited healthcare professional DESMOND Educator.

• To work as part of a team to effectively deliver the DESMOND ND/F modules according to the curriculum and using the appropriate resources.

• To be prepared to study (e.g. using development workbook, curriculum and training materials and preparing for sessions).

• To carry out self-evaluation after delivery of every DESMOND education course.

Essential criteria
  • GCSEs / O Level or equivalent
  • Computer Literate

Experience

Essential criteria
  • Previous experience of working with groups (paid or voluntary)
Desirable criteria
  • Formal training related to either teaching, counselling or group leadership.
  • Attended/observed a DESMOND/Expert Patient Programme (EPP)/similar course

Skills & Knowledge

Essential criteria
  • Demonstrate respect & value of the contribution of others.
