Job overview
The London Ambulance Service (LAS) is the largest ambulance service in the UK and plays a critical role in delivering high-quality urgent and emergency care across London. Through the Integrated Care and Coordination (ICC) Hub, LAS is working collaboratively with North West London (NW London) system partners to ensure patients access the right care at the right time.
The NW London ICC Hub brings together senior clinicians, LAS staff, and system partners to deliver improved patient outcomes, reduce unnecessary conveyance to emergency departments, and strengthen coordination with community, primary care, and hospital services.
Main duties of the job
The ICC Hub Senior Clinical Decision Maker (SCDM) will be responsible for leading clinical decision-making within the Hub. This includes assessing referrals, supporting LAS clinicians through clinical handovers, and working as part of a multidisciplinary team (MDT) to ensure patients receive the most appropriate clinical outcomes. At high level the SCDM will be involved in:
· 999 Stack review and patient assessment
· HCP/IFT admission assessment
· Care Home intervention
· Clinical Advisor decision support
· On scene clinical decision support
· Facilitating pathway referrals
The post holder will provide direct patient consultations via telephone and video, liaise with healthcare professionals (HCPs) accessing the 999 system, and ensure safe and effective transfer of care across services. They will also contribute to service evaluation, governance, and pathway improvement across urgent and emergency care.
This post will be employed through LAS under the London Staff Movement Agreement. LAS will retain clinical responsibility for the patient.

Person specification
Education and Qualifications
Essential criteria
- Full GMC Registration and License to practice
- FCEM or equivalent
- MBBS or equivalent
- Attained CCT or CESR accreditation (or be within 3-6 months) in Emergency Medicine before the start date for this post.
Experience
Essential criteria
- Provider in at least two of ALS, ATLS, APLS, MIMMS
- Instructor in at least one of ALS, ATLS, APLS
- Use of Ultrasound in EM
Desirable criteria
- Research experience with publication record
- Child Protection Awareness
