Job overview

A vacancy has arisen for enthusiastic and flexible Band 7, District Nursing  Team Managers to join community nursing teams across the Boroughs of Richmond and Hounslow

The District Nursing Team Manager is responsible for the management of a caseload of patients, including case management of patients identified as being at risk of hospital admissions. Provides skilled, professional holistic assessment of health needs, relevant nursing care and health information to members of the community of all age groups.

The Team Manager works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.

Main duties of the job

To provide clinical leadership and management responsibility to the Community Nursing team in order to meet the needs of the caseload and local population.The Team Manager will line manage a team of skill mix staff and be  responsible for the development and competencies of staff within their team.

To be responsible for the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.

To work in partnership with the primary health care team, social services and the wider multidisciplinary locality team, providing holistic assessment of health needs and the provision of nursing care to housebound people with a range of medical needs and long term conditions.

To work flexibly across teams and/or transfer between teams to meet service needs.

Qualifications and Knowledge

Essential criteria
  • Qualified RGN with current registration with NMC
  • Diploma or BSc (Hons) Community Health (DN)
  • Relevant post registration study at diploma/degree level and relevant CPD e.g. palliative care, tissue viability
  • Nurse Prescriber
  • Mentorship course
Desirable criteria
  • Independent Prescribing
  • Physical Assessment Skills
