We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Job overview

An excellent opportunity has arisen for an experienced and motivated Nurse to join our Hounslow Community Cardiorespiratory and Post-COVID Service, based at the Heart of Hounslow.

Candidates must be NMC registered with substantial experience in respiratory or cardiac care, and demonstrate the ability to work autonomously as well as collaboratively within a community setting.

Main duties of the job

The post holder will play a pivotal role in the assessment, management, and rehabilitation of adults with complex respiratory, cardiac, and post-COVID conditions across community, clinic, and virtual settings. 

Key areas of responsibility include delivery of Pulmonary and Cardiac Rehabilitation programmes (both face-to-face and virtual), airway clearance and breathlessness management, as well as oxygen therapy assessment and prescription in accordance with BTS and NICE guidance. The role also encompasses case management of patients with long-term respiratory and cardiac conditions, post-COVID rehabilitation, and contribution to the Respiratory Virtual Ward and Early Supported Discharge pathways.

You will work closely within an established multidisciplinary team, including nurses, occupational therapists, psychologists, and consultants, ensuring a seamless and patient-centred approach to care across community and acute interfaces. The role requires strong communication, advanced clinical reasoning, and leadership skills, with opportunities to contribute to service development, clinical education, audit, and quality improvement initiatives.

Essential criteria
