West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

1     The West London NHS Trust is seeking a fixed term Specialty doctor to join the Hammersmith & Fulham older adults inpatient Team (Meridian ward). It is multidisciplinary inpatient team with a consultant, CT doctor and specialty doctor, OT, SALT, Psychologist, physiotherapist and mental health nurses and Health Care assistants. It includes assessment of mental health problems and provision of effective, evidence-based treatments. The service is accessible and relevant, non-discriminatory, respects cultural values, and aims to be user friendly and user centred for patients and carers. The service intends to advocate for patients where necessary and will ensure that care is delivered in the least restrictive and least disruptive manner possible. Meridian ward is an admission ward for older adults 65+. It predominantly looks after patients with a functional diagnosis. There will also be times, often due to bed pressures, where older adults with a diagnosis of dementia and challenging behaviour due to neurodegenerative diseases are admitted onto Meridian Ward to ensure flow throughout the inpatient wards.

Undertake medical assessment, diagnosis, treatment and care of patients admitted to Meridian ward.  Completion of capacity assessments for treatment and admission is a necessity.

 

Provide support, advice, liaison and information to all who might be concerned in the care of the patient. This includes education and advice to family members/carers of patients as well as professional carers and professionals who are involved with the patient’s care

 

Work alongside members of the inpatient teams

.

Attend weekly multi-disciplinary team meeting

 

Have a good understanding of, and work in close liaison with, colleagues in SOAMHS, CIDS community teams, MINT services and other organisations.

 

Maintain confidential up-to-date patient and other records and provide letters, reports and information on clinical and other activity

 

Interact with patients and all colleagues in a courteous and supportive manner.

 

The post holder will not be supervising any other clinicians

  • Good standing with GMC with respect to warning and conditions on practice

  • Previous experience within Psychiatry (inpatient or community)
  • Previous experience within the NHS

Clinical knowledge

  • Good clinical skills and understanding of psychiatric assessment and management

  • Ability to undertake risk assessments
  • Good communication skills
  • Ability to work effectively within an MDT
  • Competent skills for contemporaneous record keeping
