Job overview

The Band 6 Social Worker / Mental Health Practitioner is a key member of the City & Hackney Crisis Resolution Service, primarily working within the Home Treatment Team, with flexibility to support other parts of the crisis pathway when required, including the Crisis Assessment Team, Homerton Psychological Medicine and Street Triage. The post holder delivers high‑quality, recovery‑focused social work and mental health interventions to adults experiencing acute mental health crises, supporting community‑based care and facilitating timely discharge from inpatient settings.

The role is clinically focused and requires autonomous practice within a 24/7 multi‑disciplinary service. Responsibilities include undertaking holistic mental health and social care assessments, Care Act 2014 care and support needs assessments and carers’ assessments, and developing person‑centred care plans. The post holder provides specialist social work expertise to the MDT, particularly in safeguarding, housing and homelessness, domestic abuse and complex social needs, acting as Lead Enquirer for adult safeguarding when required.

The role involves flexible shift working, supervision of junior staff, and active contribution to service development, quality improvement and safe, compassionate crisis care.

 

Main duties of the job

Undertake comprehensive mental health and social care assessments for adults in crisis, including mental state examinations, risk assessments and development of management plans.
Deliver timely crisis interventions within the community and Home Treatment Team setting to support individuals to remain safely at home and prevent unnecessary hospital admission.
Act as Lead Enquirer for adult safeguarding, undertaking safeguarding enquiries, developing protection plans and representing the service at statutory safeguarding and multi‑agency meetings, including MARAC.Provide specialist social work advice within the multi‑disciplinary team, championing the social model of care and addressing complex social issues including housing and homelessness, domestic abuse, carers’ needs and social inclusion.
Support safe and effective discharge planning from inpatient settings, working collaboratively with ward teams, local authorities and partner agencies.
Work flexibly across a 7‑day rota, including nights, managing unpredictable and high‑risk situations using professional judgement and adherence to Trust policies.
Supervise and support junior staff, including support workers and peer support workers, and contribute to internal training, professional development and service improvement.
Maintain accurate, timely clinical records and ensure compliance with professional standards, safeguarding legislation and Trust governance requirements.
