This exciting and innovative post has been developed as there is increased recognition for Specialist Respiratory Support within our community services, to work collaboratively with the Trust, department and community partners to lead, develop and support patients with acute and chronic respiratory diseases.

The successful applicant will join our dynamic respiratory department as a Consultant with a special interest in General Respiratory Medicine and Integrated care. The post includes 10 PA of activity. 

The successful applicant will work closely with our multidisciplinary community respiratory team; the Adult and Respiratory Care and Rehabilitation Team (ARCARE). The ARCARE team already provides continuing care for patients with complex COPD, chronic breathlessness, diagnostic spirometry, home oxygen assessment and review services (HOSAR), palliative care, psychology and pulmonary rehabilitation. The service also provides admission avoidance and supported discharge to COPD patients, and is supported by a fortnightly Consultant-led multi-disciplinary meeting. 

Main duties of the job

1.    To contribute to the provision of a comprehensive and efficient acute and specialist Respiratory Medicine service at Barts Health NHS Trust.

2.    To work closely with colleagues in both primary and secondary care, to provide seamless, integrated care for patients with chronic respiratory conditions and acute exacerbations.

3.    To provide evidence-based care for all inpatients and outpatients.

4.    To develop community partnerships within the community virtual wards across north east London. 

5.    To have regard at all times for the clinical and quality standards.

6.    To liaise effectively and on a timely basis with General Practitioners, community services, and all healthcare agencies, and to work collaboratively across North East London as required.

7.    To maintain and promote team and multi-disciplinary work with the specialties working closely and regularly with the consultant medical staff.

8.    To participate fully in the management of the Department and to liaise closely with the General Manager and the Clinical Director.

9.    To take an active role in teaching and training in the department and Trust at both undergraduate and postgraduate level.

Detailed job description and main responsibilities

The post holder will work with the existing team and consultants to help implement the Community Virtual Wards Acute Respiratory Infection (CVW-ARI) across Tower Hamlets, to provide a safe and efficient alternative to NHS bedded care and allow patients to receive the acute care they need in their own home. They would also be expected to help deliver multidisciplinary respiratory meetings and teaching across primary care

The secondary care component of the job plan would include the delivery of comprehensive, consultant-led Respiratory care for inpatients at the Royal London, sharing of a weekly bronchoscopy list, outpatients including Respiratory ‘HOT’ clinics and supporting Allied Health Professional clinics, both based at Royal London or Mile End Hospitals, and would join a weekly consultant rota with inpatient ward rounds and Respiratory on-call cover for the Royal London and St Bartholomew’s Hospitals.  There is no GIM on-call commitment

Qualifications

Essential criteria
  • GMC Registration
  • MBBS or equivalent
  • MRCP or equivalent
  • CCT in Respiratory Medicine and General (Internal) Medicine, G(I)M, (or within 6 months of eligibility at the time of interview) or be on the Specialist Register
Desirable criteria
  • MD or PhD

Relevant Experience

Essential criteria
  • Evidence of broad respiratory and general medicine training.
Desirable criteria
  • Experience in integrated health or specialist COPD or other tertiary chronic lung disease experience
