Job overview

The GP Clinical Lead for Bedded Rehabilitation will provide medical care and clinical leadership across Athlone and Alexandra Rehabilitation Units, located 1.2 miles apart (W9 and W10). Working as part of a multidisciplinary team, the postholder will support the rehabilitation of adult patients—predominantly older adults stepped down from acute care—helping to deliver high-quality, person-centred rehabilitation services.

This role offers experienced GPs the opportunity to work in a supportive multidisciplinary community healthcare environment, expand skills in complex care, and liaise across organizational boundaries to support integrated patient pathways.

Main duties of the job

Clinical Responsibilities

  • Lead the medical care of patients on the rehabilitation units.
  • Liaise with colleagues to ensure patient suitability for admission.
  • Develop and implement individualized medical care plans for newly admitted patients, including seeking specialist input where appropriate.
  • Provide medical leadership at MDT meetings and during ward rounds to ensure every patient is reviewed at least weekly.
  • Complete patient documentation, discharge summaries, and letters in line with GMC standards and Trust policies.
  • Ensure accurate prescribing and completion of drug charts in accordance with local guidelines.
  • Review deteriorating patients promptly and escalate appropriately following Trust protocol.
  • Provide expert clinical advice to medical, nursing, and therapy colleagues.

Essential criteria
  • Full GMC registration
  • Certificate of Completion of Training in General Practice

Experience

Essential criteria
  • Experience of working across organizational boundaries in healthcare
  • Experience of developing clinical governance
Desirable criteria
  • Work as a health professional in a community setting, including GP practices

Skills & Knowledge

Essential criteria
  • Good understanding of older adults, acute medicine, frailty, and rehabilitation
  • Working knowledge of Microsoft Word and GP practice systems
  • Ability to interpret complex or incomplete information
  • Able to influence and persuade others articulating a balanced view and able to constructively question information.
  • Able to build relationships with a range of stakeholders which are based on openness, honesty trust and confidence
