Job overview
Advanced Nurse Practitioner Band 8a required for the Barnet H@H service .
The H@H service supports the early discharge home of patients on the Frailty Pathway, patients are on the Hospital at Home ward from 1-14 days. Initially the service is for patients requiring IV antibiotic, however we will be stepping up to see patients who require monitoring of observation, COPD monitoring, heart failure, and blood taking.
The virtual ward operates from BGH and RFH. We envisage long term we will also receive patients from 111/999 and step-up patients from the RR and D2A pathway.
Main duties of the job
- Be clinically and operationally responsible for the team Act as a role model.
- Be responsible for undertaking comprehensive health assessments, physical examination and ordering tests for patients.
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population.
- Be involved in establishing delivering and leading a comprehensive service within community services.
- You will play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and through networking increase the number of referrals from the acute and community.
- Lead on service development.

Education/Qualification
Essential criteria
- RGN / RN or AHP with current UK NMC registration or other registration body.
- Educated to degree level.
- Appropriate postgraduate qualification or experience in relevant clinical area.
- Non – medical prescriber qualification required
Desirable criteria
- Qualified MSc in a relevant speciality with leadership module
Experience
Essential criteria
- Signficant post registration experience at band 7.
- Demonstrable experience of working in a community setting
- Demonstrable experience in a range of long-term conditions / frailty / patients with comorbidities
- Postgraduate experience of working in a multi professional team.
- Experience of teaching and training.
- Experience of research and clinical audit
- Experience of clinical management
- Experience of managing staff performance and addressing competency issues.
- Experience of minor adaptations and home modifications
- Experience of carrying out audits and the formulation of action plans.
- Experience in setting up new projects /initiatives and leading change management
Desirable criteria
- Experience of working with social services
- Experience of successful project management within a clinical area
