Are you a Band 3 with 2 years experience or a Band 4 Rehabilitation Assistant/ Care or Support Worker looking for a Band 4 Rehabilitation Support Worker role in the community? If the answer is “yes”, we would love to hear from you!

We are looking for highly motivated, passionate and enthusiastic team players with a positive attitude and commitment to providing excellent holistic care and interventions. You will be able to be creative and resourceful in your approach to providing rehabilitation using your skills to assess, support and carryout rehabilitation plans for a varied caseload of patients.

This job is based at East Barnet Health Centre, and will be seeing patients in the borough of Barnet in their own homes or in the clinic settings.
Barnet planned care therapy team comprising of Occupational Therapists,
Physiotherapists, District Nurses, Case Managers, Pathway Support Worker, Rehabilitation Care Workers (RCW), Technical Instructors and Administrators

Main duties of the job

Barnet Planned Care Therapy team provides rehabilitation to adults over 18 years of age, working with them to regain their independence following illness or injury including falls, preventing avoidable attendance at and admission to hospital, supporting them to manage their long term conditions and promoting health and wellbeing.

The Rehabilitation Support Worker will assist with therapy interventions and programs such as home exercise programmes, clinic based strength and balance exercise programmes, mobility/ stairs practice, carrying out initial risk and V1 updated 13th Jan 2012 functional assessments, providing aids/ minor adaptations, training patients with their personal and domestic activities of daily living to promote independence and
safety, under the guidance and supervision of qualified therapists and or nurses.

You will manage a caseload of non-complex cases with minimal supervision by the therapy staff delegating tasks or setting up the rehabilitation programmes.

You will be carrying out specific Occupational Therapy, Physiotherapy and nursing interventions and be responsible for collecting a range of information on clients’ needs and health status. Other duties such as clerical and administrative tasks as required.

Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)would be desirable.

Detailed job description and main responsibilities

  • To review caseload on a regular basis with referring staff and clinical supervisor and report back appropriately when goals achieved/not achieved and if concerns are raised, or new information becomes available which impacts on intervention provided.
  •  Input into local assessment documentation and reports under the guidance of qualified staff.

Education/Qualification

Essential criteria
  • Good general education (GCSE or equivalent knowledge)
  • NVQ Level III
  • Good oral and written communication skills, basic computer and keyboard skills.
Desirable criteria
  • Willingness to undertake further training as appropriate
  • Trusted Assessor trained.
  • PSI qualification

Experience

Essential criteria
  • Minimum of two years’ experience as a Therapy/ Rehabilitation Assistant working in health or similar post.
  • Experience of working in the Community setting
  • Experience of working with elderly people and in particular frail elderly people and people with physical disabilities.
Desirable criteria
  • Experience with SystmOne

Skills and Knowledge

Essential criteria
  • Knowledge of the role of a Rehabilitation Assistant in a community rehabilitation team
  • Basic knowledge of the role of Allied Health Professionals (e.g., Occupational Therapy, Physiotherapy) in the treatment of physical conditions
  • Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
  • Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
  • Able to problem solve, organise, prioritise and demonstrate sound judgement
  • Basic competency training in: Mobility assessment, individual exercise programme, falls prevention.
  • Experience in assessment of Personal and Domestic Activities of Daily Living and carrying out these tasks with clients to improve ability
Desirable criteria
  • Basic knowledge of common medical conditions and a basic understanding of their impact on functional ability
  • Knowledge and experience of using outcome measures appropriate for fallers.
  • Basic knowledge and understanding of the National Service Framework for Older People
  • Basic knowledge and understanding of the role of Social Services and voluntary agencies in community rehabilitation, and processes for referral.
  • Basic presentation skills appropriate for teaching students and peers in an in-service training setting

  • Able to demonstrate excellence in Customer Service.
  • Able to meet the required IT Skills for the post.
Desirable criteria
  • Able to provide evidence of where you have demonstrated the Trust’s Values and Behaviours.
