Job overview

Camden Rapid Access Services (RAS) is a Physical health admission avoidance service.

Camden RAS covers the entire borough of Camden, this role is predominantly for Urgent Community Response however you may be asked to cross cover the Virtual ward on occasion.  Our overall aim is to avoid unnecessary hospital admissions, as part of the NHS Long term plan. Main duties of the job

This post will support the team by providing appropriate clinically-based pharmaceutical services to patients with long term or complex medication needs. The post holder will conduct domiciliary visits and complete technical medication reviews to support patient’s compliance problems. This will include full medicines reconciliation, counselling patients about their medicines and coordinating the supply of medication.

Detailed job description and main responsibilities

  • Liaise with patients, community nursing and social care teams, GP practice staff, GP’s, GP practice pharmacists, community pharmacists, care home staff, falls prevention team and secondary care discharge co-ordinators and pharmacy teams in all aspects of technical medication compliance reviews.
  • Attend staff meetings/handovers to identify patients suitable for medicines optimisation technician intervention.
  • Support the implementation of changes required and agreed by the GP with regard to dosage, formulation of product, supply of compliance aids.
  • Record actions and recommendations in patients’ notes/clinical management plans and inform their GP.
  • Provide clinical advice and training to patients/clients and carers/staff in accordance with evidence-based guidance e.g. NSF’s, NICE Guidance or Local guidelines.
  • Ensure effective communication to patients about changes in how their medicines management needs can be best met and document it in the care plan.
  • Prepare reports from the technical review and feedback to the relevant members of the healthcare team.

Person specification

PREVIOUS EXPERIENCE

Essential criteria
  • Hospital/GP practice pharmacy work experience
  • Experience of working on wards, counselling patients, checking PODs, and medicines reconciliation.
  • Liaison within a multidisciplinary team
  • Experience of supporting or developing pharmacy technician services.
  • Portfolio demonstrating competency of previous and current pharmacy practice skills, knowledge, and CPD.
Desirable criteria
  • Experience working within care of elderly services
  • Working in an integrated care (or equivalent) setting
  • Post qualification experience of a medicines optimisation role to care of the elderly
  • Primary care pharmacy work experience
  • Experience in a supervisory capacity

EDUCATION AND QUALIFICATIONS

  • BTEC/NVQ Level 3 in Pharmaceutical Sciences plus registration with GPhC
  • HEE Medicines Optimisation Programme (MOP) or equivalent qualification
  • Practical experience and theoretical study / knowledge (as evidenced by CPD) in Medicines Management
Desirable criteria
  • Medication Counselling Training
  • NVQ Workplace Assessor Award
  • Membership of APTUK
  • Accredited Checking Pharmacy Technician (ACPT)
  • Practice Supervisor qualification
