Chelsea and Westminster Hospital NHS Foundation Trust has been commissioned to provide TransPlus at 56 Dean Street, the first integrated gender, HIV and sexual health service by NHS England. Our vision is for an innovative and patient-centred service, based on the principle that patients must have equal access to services whatever their gender identity with reasonable adjustments to the delivery of care to match the individual’s needs and circumstances.
We are looking to recruit an experienced Senior Nurse with extensive experience in working in Gender Identity Care to join our team as a Nurse Practitioner. You will have passion and empathy for patients and a strong desire to enhance patient outcomes and develop the Nursing workforce in Trans Plus through training and education both theoretically and clinically based work. You will be Job Planned to see patients Clinically 60% of the time.
Main duties of the job
The post holder will work within the Gender Identity Service (Trans Plus) based at 56 Dean Street providing Clinical assessment and diagnostics to a select patient cohort throughout the working week as well as provide education and training junior staff and allied health workforce. You will work clinically with all nursing staff team members of all Bands to ensure that continuing personal, professional, & practice development is an active process, & that high quality nursing care is delivered in line with current evidence based practice. You will provide clinical leadership to staff within the area, working in close collaboration and co-operation with members of the multi professional team and outside agencies. You will ensure all activities in the area are carried out within the Chelsea and Westminster Hospital Foundation Trust, and Directorate policy guidelines. You will assist with and develop service provision for patients accessing the Gender Identity service at 56 Dean Street and be responsible for audit and research required for the service and present relevant data.

Clinical
· To be an expert practitioner, demonstrating advanced communication and nursing skills, using expert knowledge and decision-making skills within Gender Identity Care focusing on assessment and follow-up care.
· To provide excellent visible clinical leadership, and develop support mechanisms for sharing good practice.
· To be flexible in the approach to the clinical areas and provide cover for any area specified within your clinical competence.
· Work strategically and operationally within the Directorate team in ensuring that clinical policies, procedures, guidelines and educational programmes meet the needs of the Directorate.
· Act as a resource, provide accurate information, advice and support in education and innovation.
· Be aware of the Trust’s safeguarding policies and procedures and demonstrate the ability to be able to enable others to raise concerns and work with the Trust’s safeguarding team.

- Registered Nurse
