The primary care services we provide vary from specialist services, providing care to vulnerable and homeless populations, to more mainstream general practice and we work across East London and Bedfordshire. The Long Term Plan sets out a vision for primary care to lead on population health improvement and we work in partnership with mental health, community services, acute services, social care, the voluntary sector and ambulance services to help our local populations stay well.

Job overview

The Pathway Homeless Team works across the Royal London Hospital in-patient and Accident and Emergency Services to provide advocacy, support and quality discharge interventions for homeless clients attending or admitted to the Royal London Hospital. The Homeless Team works in close collaboration with the wider hospital discharge teams.

The dual aims of the Homeless Team are to improve the quality of care for homeless patients, whilst reducing potential delayed and also premature discharges. There is an overarching aim to reduce future unscheduled admissions and A&E attendances.

The Homeless Team staff team is a multi-disciplinary and multi-agency team, and is currently in development. The ongoing team includes 1 Clinical Lead GP, 1 Band 7 Nurse, 1 Band 6 Nurse or Occupational Therapist, 1 Band 6 Social Worker (seconded from Barts Health), a care navigator and 1 part-time Band 4 Administrator.

Main duties of the job

Please note that the post holder must be available to work on Wednesdays & the job is in person not remote working

The Administrator is responsible for organising and coordinating office operations and procedures in order to ensure organisational effectiveness and efficiency for the Homeless Team on a day-to-day basis.
                     

The post-holder will also play a key role in supporting the team to manage the team budget effectively, and report on their service outcomes in an accurate and timely manner.

The post holder will also provide high quality administrative support service to individuals.

Effective internal and external liaison will be required to contribute to the successful running of the service as a whole.

·     The post holder will have regular contact with all members of the multi-disciplinary team and be required to liaise regularly with wards and departments throughout RLH. 

·      The post holder will also be required to liaise regularly with outside organisations including health and social care providers, local authority, government agencies, and voluntary sector services.

  • There will be some contact with patients, although this will mostly be on the phone.
  • Take messages and ensure they are passed on promptly and accurately to the correct person in accordance with Homeless Team policy
  • Experience with using the following clinical systems: Cerner, and EMIS is ESSENTIAL
