Job overview
This is a fixed term community mental health role within Tower Hamlets Community 24/7 service pilot. The pilot ensures delivery of high quality, modern, patient-centered services. This is a pilot project aimed to establish and test out a new model of care that breaks down current barriers to care, is nonrestrictive, co-produced, culturally sensitive and accessible. The service will run 24hrs a day, 7 days a week for people residing in the catchment area offering open access to a range of psycho-social- medical Interventions at every stage of their wellness journey, this includes short term Hospitality stays for those in crisis, and community based offers that are embedded in the neighbourhood.
The Team is multi-disciplinary and multi-agency and committed to co-production, facilitation and liaison with service users, local residents, local VCSE, health and social care services in order that people with co-morbid physical and mental health conditions, experience good outcomes for their mental health. Additionally the team provides specialist input where this is required.
Main duties of the job
· Welcome all attending the community hub within its values and ethos
· Make sure that co-production is at the core of the role
· Ensuring that compassion and empathy are central to care delivery
· Ensuring that the local demographics are taken into consideration for service provision and improvement
· Participating in all MDT working eg meetings, visits, assessments and interventions
· Making Contact with the service user in a timely Manner following from Triage to screen for risk effectively and to complete initial assessment.
· To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
· To promote a service user focused service.

We will work in partnership with Look Ahead Housing Association, the Tower hamlets VCSE Alliance, experts by experience, in a new fully integrated community mental health centre based in Bethnal Green and serving PCN1.
We are one of six pilot sites in England and intend to reshape care.
We are looking for a person who has a good understanding of the Trieste model and is keen on being part of a new way of working. Integral to the role and ethos of the service is a commitment to co-productivity and the eradication of a “them-and us” culture, a removal of barriers that hinder good care, ensure compassion, empathy and relationship building as the primary focus.

Education/ Qualification/ Training
Essential criteria
- • Pre-graduate mental health nurse in the final six months of their training or a graduate mental health nurse who is pending their PIN or a Registered Mental Health Nurse
- • Willingness to undertake further training relevant to position
Experience
Essential criteria
- Experience of mental health nursing either in inpatient or community setting
- • Working with people with mental ill health /learning disabilities and complex emotional needs
- • Experience in de-escalating distressed people
Knowledge
Essential criteria
- • A clear understanding of co-production Knowledge of the Mental Health Act (1983), Mental Capacity Act (2005) & the Care Act (2014.
- • Understanding of the principles of the recovery model, DIALOG+
- • Up to date knowledge of the current approaches to mental health nursing
Desirable criteria
- • Knowledge of risk formation and ability to teach others
Skills
Essential criteria
- • Able to work sensitively and compassionately with patients
- • Ability to assess, plan, implement and evaluate care using evidence based approaches
- • Ability to prioritise workload.
- • Good IT skills.
