Job overview
Telehealth is the means by which technologies and related services concerned with health and wellbeing are accessed by people or provided for them irrespective of their location. 
ELFT’s telehealth services are offered by its Telehealth Team. The services provide care for people with COPD, diabetes, heart failure and hypertension and are based on a step up/step down model. The Telehealth service is provided to local service users and integrated with the wider community services and other stake holders i.e. primary care and secondary care services aiming to deliver the right quality care at the right place, at the right time including digital. 
Main duties of the job
Some of the main responsibilities of the role are
Assessing patients and reviewing the care plans with a view to enrolling onto the Telehealth systems and continue remote monitoring using technologies.
To provide initial and ongoing holistic assessment of nursing needs of patients and carers in the community. Able to work as an autonomous practitioner in the assessment, integrated planning and review of programs for Telehealth patients.
Monitoring and being responsible for the work activities and nursing care standards of the TH Healthcare support workers.
Support Urgent Care Response, Extended Primary Care Team and community nurses in the delivery of nursing care to patients when appropriate and required.

Detailed job description and main responsibilities
Facilitating early discharges from the acute sector as appropriate to the 
Extended Primary Care Team and Rapid Response. 
Monitoring caseloads in conjunction with team members. Discussing with the ANP / Clinical Leads any concerns or significant changes in the case load and also deputizing the Telehealth advanced practitioner as and when required.
