Job overview
We are seeking for an experienced and innovative Occupational Therapist to lead and shape our Reablement Service, supporting individuals to regain independence, reduce hospital admissions, and optimise recovery across an integrated health and social care system.
The post holder will operate at the forefront of community transformation, shaping pathways that prevent hospital admissions, accelerate discharges, and deliver person-centred care at home. You will influence practice beyond your immediate team, working across system partners to design service around the needs of complex, diverse population.
You will lead a dynamic, multidisciplinary workforce and drive a culture that is proactive, responsive, and outcomes focused, where reablement is not just a service, but a system wide approach to care.
A key part of the role is developing the future workforce. You will provide clinical leadership and structured development for Band 5 OTs on rotational programmes, supporting them to build confidence, competence and a strong foundation in community rehabilitation and reablement practice.
Main duties of the job

Provide strategic and clinical leadership for the Reablement OT Service.

Lead on complex case management, including frailty, falls, and multi-morbidity.

Drive service development, transformation, and quality improvement initiatives.

Work collaboratively across health and social care to streamline pathways.

Support workforce development, supervision, and clinical education.

Contribute to system wide initiatives including admission avoidance and discharge flow.

Detailed job description and main responsibilities
To clinically lead and develop the provision of a high quality expert Occupational Therapy Service to a specialist community caseload of adults and older people with a wide variety of disabilities and their families/carers.
The post is community-based within an integrated multidisciplinary Reablement Service.
It involves the training, support and guidance of a range of staff to undertake enhanced roles, including personal care, rehabilitation and reablement so that a more holistic/integrated approach to the management of Service Users with complex needs (those with chronic illness / disease) can be achieved and the users’ independence is promoted.
It also involves the assessment and provision of occupational therapy, working with users and informal carers in their homes and local communities to promote independent living.
The post holder will be responsible for the clinical and professional supervision, training & development of all staff working within the Reablement Service.

Experience
Essential criteria
- Appropriate post qualifying experience in NHS or similar health service
- Experience of Service redesign and implementing change
- Previous participation in research
- Experience of equal opportunity policies and procedures including the needs of a diverse community
Qualifications
Essential criteria
- BSc degree in Occupational Therapy
- HCPC Registration
- Evidence of ongoing, relevant postgraduate education including leadership training
Desirable criteria
- Evidence of relevant masters level qualification or equivalent specialist experience
Knowledge and Skills
Essential criteria
- Evidence of knowledge in legislation, health and safety policies and procedures of both the local authority and NHS trust
- Knowledge of emerging social care and NHS strategy and policy
- Knowledge and application of evidence based practice
