Job overview
We are looking for an experienced Advanced Nurse Practitioner (ANP) to join our dynamic and supportive Urgent Community Response (UCR) Team. This is a fantastic opportunity to deliver high-quality, patient-centered care in a fast-paced team.
As an Advanced Nurse Practitioner in the Urgent Community Response (UCR) Team, you will play a critical role in delivering high-quality, person-centred care to patients experiencing acute illness or deterioration in the community. You will act as an autonomous practitioner, leading rapid assessment, diagnosis, and treatment plans to prevent avoidable hospital admissions and support early discharge from secondary care.
Act as a role model and provide:
- Clinical leadership
- Supervision to junior colleagues
- Promoting clinical effectiveness
- Share knowledge with others member of the Multi-Disciplinary Team (MDT).
The ANP will work closely with all members of the MDT and will play an integral role in assisting in the operational/strategic development of and the proposed frailty service; promoting and facilitating evidence-based practice, clinical audit and research in conjunction with the Clinical Lead ACP and Deputy Lead Nurse. The post holder will care for a wide range of patients from varying socioeconomic back grounds
Main duties of the job
Key Responsibilities:

Provide advanced clinical assessment and management of patients referred to the UCR team, including those with complex, multiple comorbidity.

Undertake physical examinations, interpret diagnostic tests, and initiate treatments including prescribing medications and interventions within your scope of practice.

Lead multidisciplinary discussions and care planning in collaboration with GPs, community teams, paramedics, and social care professionals.

Respond rapidly to referrals within the 2-hour UCR response time frame, ensuring timely triage and intervention.

Support hospital discharge pathways by assessing patients' ongoing care needs and coordinating appropriate community services.

Contribute to service development, audit, and quality improvement initiatives to enhance patient outcomes and service efficiency.

Provide clinical leadership and mentorship to junior staff and students, promoting evidence-based practice.

Detailed job description and main responsibilities
Key Responsibilities
The ANP will:
1. The post holder is expected to hold a qualification in Advanced Clinical Assessment skills and Non-Medical Prescribing (V300) with the ability to practice a high standard of professional conduct in line with the policies, protocols and guidelines of the Trust and NMC and Standard documents, ensuring that other members of the nursing team are aware of them.
2. Developing quality initiatives in standard setting and auditing programmes of care. Participating in research projects as required.
3. Work with clinical teams across CHN providing clinical care to a wide breadth of patients in line with their scope of professional practice which will continue to expand.
Main Duties and Responsibilities
Clinical
- Take a leading role in the enhanced management of patients who require enhanced care/treatment to prevent a hospital attendance.
- See patients in their own home/usual care environment as an autonomous practitioner (within scope of practice).
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undiagnosed and undifferentiated illness (within scope of practice).
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly (within scope of practice).
- Proactively identify, diagnose and manage treatment plans for patients at risk of deterioration outside of the hospital setting.
- Facilitate and support multidisciplinary team working with internal and external colleagues to aid clinical decision-making and promote patient safety.
- Refer onto specialist services if necessary
- Liaise with diagnostic services if patient requires further investigations.
MANAGEMENT
- Support staff development in order to maximise potential.
- Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice
- Critically evaluate and review innovations and developments that are relevant to the area of work.
- Enlist support and influence stakeholders and decision-makers in order to bring about new developments in the provision of services.
- Lead clinical teams and ensure support structures are in place for the smooth running of relevant services.

Education/ Qualification/ Training
Essential criteria
- • Registered nurse/paramedic
- • V300 Non-Medical Prescribing
- Advanced Clinical Assessment Skills
- • BSc in related subject
Desirable criteria
- • MSc in Advanced Clinical Practice (or willing to undertake).
Experience
Essential criteria
- • Minimum of 2 years’ experience in primary/ urgent/ emergency or unscheduled care.
- • Extended scope skills relevant to unplanned, out of hours, community care
- • Leadership of teams, Clinical governance, quality and safety experience
- • Minor illness/ailments experience with triage/prioritisation
Desirable criteria
- • Project management/ research and evidence-based practice
Knowledge and Skills
Essential criteria
- • Advanced understanding and experience of clinical governance and leadership
- • Exemplary written, verbal communication skills and negotiation skills
- • Management and supervisory experience
- • Peripheral venepuncture and cannulation competency
Desirable criteria
- • Experience of interpreting basic blood results (FBC/U&E/LFT/CRP/ ABG)
