An exciting opportunity has arisen to lead the next phase of the City and Hackney neighbourhoods programme.
We have established 8 Neighbourhoods across City and Hackney which each serve populations of 30,000 to 50,000 people. Our Neighbourhoods provide the structure to address health inequalities and deliver preventative, joined-up care utilising a population health management approach working in partnership with local communities. The Neighbourhood model is a key strategic objective for City and Hackney health and care partners as we recognise that this way of working is required to meet changing demographic pressures and support sustainability of our health and care system.
Having successfully established supporting infrastructure, enablers and ways of working, we are now transitioning to fully embed Neighbourhood teams as standard practice across our place to meet changing demographic pressures and support sustainability.
The post holder will have a clinical or practitioner background, coupled with leadership and operational management experience. They will draw on this experience to lead a complex programme to design and implement our City and Hackney Neighbourhood team model, working in partnership with the local authority, voluntary sector and community organisations, patients, carers and the wider community in City and Hackney.
Main duties of the job
Leading the development and implementation of the Neighbourhood team(s) model across City and Hackney, utilising clinical/practitioner expertise to ensure models are grounded in sound professional practice and meet service user needs
Applying professional knowledge and experience to establish operational Integrated Neighbourhood Teams with appropriate clinical/care pathways across primary care, community health, mental health, social care, and voluntary sector partners
Drawing on practitioner insight to develop and implement standard operating procedures and practical tools that enable multidisciplinary teams to deliver coordinated care effectively
Using professional credibility to influence and secure buy-in from clinical and practitioner colleagues across all partner organisations in the delivery of integrated Neighbourhood care
Utilising professional experience to identify and address operational barriers that impede effective multidisciplinary working
Ensuring the Neighbourhood model delivers both preventative and joined-up care that meets the needs of people with rising risk and complex conditions, working with the clinical lead to implement appropriate structures
Working collaboratively with all key partners and using data platforms to enable population health management approaches that drive measurable improvements in outcomes

Essential criteria
- - Clinical or practitioner qualification in a relevant area of practice- for example community health services, social care or community mental health
- - Post–graduate degree or equivalent management qualification – or management experience equal to such a qualification
- - Evidence of ongoing personal development
Experience

- - Experience of working in a clinical or practitioner team
- - Experience of leading a clinical or practitioner team
- - Considerable experience of working in a complex, multi-agency environment and across organisational boundaries
- - Successful complex change management experience, with a proven track record of working collaboratively with a range of organisations to deliver programme goals
Knowledge
