We are seeking a Proactive Care-Coordinator Team Supervisor to lead and support our Care Coordinator team. This is an exciting opportunity for an experienced and motivated leader to drive service development and ensure high-quality, patient-centred care.

In this role, you will manage the day-to-day operations of the team, providing supervision, appraisals, and ongoing staff development. You will foster a supportive, positive environment that promotes teamwork and continuous improvement. A key part of the role will be to shape and develop the service, ensuring best practice and innovative approaches to meet patient needs.

We are looking for someone with experience in staff supervision and team leadership, strong organisational and communication skills, and a commitment to developing both people and services. Experience within health, care, or community services would be an advantage.

Main duties of the job

  1. Supporting the referral pathway, Identifying residents who would benefit from an Anticipatory Care approach;
  2. Aid the development of the proactive care coordination service with he support from the head of service
  3. Be a core part of Neighbourhood Team MDTs and huddles
  4. Will use effective judgement on the nature of the audit/data returns and data queries, escalating issues to senior team members.
  5. Develop systems for improving the collection of accurate and timely source data.
  6. Will take responsibility in the preparation of detailed reports, including standard weekly, monthly, quarterly and a large number of ad-hoc reports.
  7. Aid the care coordinators to, carry out their role – acting as a trusted contact that supports residents to connect with the holistic service
  8. Keep good records and keeping the Neighbourhood MDT (including relevant GP Practice) informed to ensure continuity and safety of care
  9. Provide supervision and appraisal for proactive care staff
  10. Manage the service on a daily basis, trouble shooting problems as they occur.
  11. Evaluate outcomes for individual residents and service as a whole.
  12. Working with others to support the above aims
