Are you looking for an exciting post in the field of adult diabetes?
We are offering a 12 months Community Diabetes Specialist Nurse position as for maternity cover.
You will be a member of the highly regarded multidisciplinary Diabetes Service, providing education and support to patients, their relatives and carers, in the community.
The post is based in the community at G.P clinics, working alongside our Diabetes Specialist Dietician and Primary care staff.
We are looking for a Diabetes Specialist Nurse who has a passion to deliver excellent clinical knowledge of diabetes into practice, empowering patients to manage their diabetes with confidence.
As a growing diabetes service meeting the needs of an expanding local population, the service provides a number of routine and specialist services, including; providing multidisciplinary clinic support, dietitian and nurse-led clinics, ante-natal and pre-conception clinic, insulin pump clinic, transition clinic, virtual clinic, CGMS, emergency reviews and managing routine and urgent ward referrals. We are also a recognised DAFNE and X-PERT centre.
As a successful candidate, you will possess exemplary clinical reasoning skills, empathy and advanced communication skills. We are committed to training, supervision and audit to improve clinical skills and ensure a high quality of service provision.
Main duties of the job
- The post holder will assist in the delivery of a specialist nursing service for people with diabetes.
- To liaise closely with defined GP practices and community nurses to ensure current recommended practice is effectively delivered.
- To ensure continuity of high standard evidence based nursing care, assessing, developing, implementing and evaluating care plans for a defined caseload of patients in secondary care.
- To manage own caseload of patients under supervision from a senior Diabetes Specialist Nurse.
- To follow up patients under supervision from a senior Diabetes Specialist Nurse, providing relevant information enabling patients and their carers to make informed choices regarding treatment.
- To refer patients presenting with complex diabetes associated problems on to a senior Diabetes Specialist Nurse or senior Diabetes Dietitian using integrated patient care pathways.
- To develop and participate in the on-going education of people with diabetes, ensuring timely referral to participate in appropriate structured education programmes.
- To assist patients and their carers to achieve individual goals to maintain physical and psychological well being
- Be fully conversant with the actions of insulin and oral therapies under agreed guidelines and protocols.
- Promote effective communication between primary and secondary service sectors to ensure continuity of care.
