The ACERS community heart failure service is tailored to the needs of the local population by providing evidence based care for patients in practice based clinics and in their homes, and working closely with the hospital team if they are admitted. The GPs are enthusiastically committed to the model and to providing excellent care networks for patients in partnership with Homerton Hospital cardiologists, health care professionals and other agencies.  You will be responsible for and lead on specific areas to ensure that patients have a seamless pathway through different aspects of the service to assist in the implementation of National Guidelines and Quality Standards for chronic heart failure care.

You will support heart failure patients within the community to optimise their heart failure treatment, support patients to manage their condition, and prevent unplanned admissions.  The post-holder, will also support the Lead Heart Failure Nurse Specialist in the day to day operational management of the service within the ACERS team.

Main duties of the job

We are looking for an experienced, enthusiastic, pro-active, self-motivated nurse to join the team as one of the key links in the chain of gold standard care.  Essential to this new role is a strong background in cardiology with relevant training and your excellent communication skills with which you will develop personal management plans and educate empower patients and carers for increased confidence in self-care. Experience of working in the community is desirable. This post will let your personal and clinical skills blossom as you set new ways of working in place to really make a difference.

  • Post registration nursing course in cardiac or heart failure care
  • Recognised teaching certificate e.g. ENB 998
  • Clinical assessment skills
