An exciting opportunity has now arisen for an individual to join the Physical conditions side of the team.

The Adult Community Rehabilitation Team is a forward thinking service that provides interdisciplinary rehabilitation to adults. The service is divided into two specialist pathways, Neurological and Physical Conditions.

The team consists of Physiotherapists, Occupational Therapists, Speech & Language Therapists, Dieticians, Clinical Psychologists and Neuropsychologists. The team is supported by a Consultant Allied Health Professional, Consultant Neurologist, as well as rehabilitation assistants and administrators. The team also regularly participate in neighbourhood MDM’s.

This post would suit an individual wishing to develop their clinical skills in community based  rehabilitation. You will be responsible for a varied caseload of clients with a range of complex long term health conditions, with the opportunity to support group-based rehabilitation classes. Our client group is clinically, economically and culturally diverse.

Main duties of the job

Therapy intervention is goal led, client-centred and flexible. The service is essentially community based; however there is opportunity to provide intervention in our onsite outpatient gym and other community based facilities alongside remote/virtual working.

You will also manage a rehabilitation assistant or a band 5 rotational physiotherapist and will be expected to work closely with the senior physiotherapists in the team to review and develop the service as whole.

  • Manage a wide and varied caseload supporting clients living at home teaching self management techniques for those living with long term conditions.
  • Provide detailed physiotherapy assessment and rehabilitation
  • Support junior members of the team 
  • Take part in service development projects

Essential criteria
  • Degree/Diploma in Physiotherapy
  • HCPC registered
Desirable criteria
  • Member of relevant special interest group
  • Relevant post graduate courses

Skills/Abilities

Essential criteria
  • Ability to assess and implement appropriate treatment for a range of patients with physical health conditions
  • Ability to manage caseload and to prioritise under pressure
  • Ability to communicate effectively with MDT and a wide variety of clients
  • Sound IT skills including word processing and use of email
Desirable criteria
