An exciting opportunity has arisen for an enthusiastic and motivated individual to join our integrated healthcare service as a Band 4 Therapy Assistant. We are looking for a candidate, who has good communication skills and the ability to motivate patients. The post holder will be working with people in their own homes following a period in hospital or who are at risk of going into hospital. You will help people complete rehabilitation programmes under the supervision of therapists within the team so you will need to be able to independently travel throughout the borough.
The Specialist Independence Assistance role involves supporting a Multi-Disciplinary Team (MDT) to enable daily living within a person’s own home, focusing on the aim of improving a person’s function through enablement and rehabilitation, promoting independence and ensuring the individuals’ safety and wellbeing.
It is an interesting, challenging and dynamic job working in the community and makes a real difference in people’s lives. Candidates with previous experience as a Rehabilitation Assistant or a similar role are particularly encouraged to apply.
Main duties of the job
Main duties include:
- Visit clients in their own home and implement individual therapeutic programmes under the supervision of the team of therapists for each client in accordance with the agreed Intervention Plan and goal setting.
- To work with adults and older adults in their own home or local community, to help improve their functional independence in everyday activities such as washing, toileting, dressing, food preparation, shopping, accessing shops and local appointments. This work is completed within a therapeutic framework that focuses on empowering people to maximise their independence and health. It includes providing physical assistance with personal care tasks.
- The post holder will respect the clients’ dignity at all times and adopt a client centred approach.
- To assist in the co-ordination of the therapy programmes and individual timetables. This can include liaising with family, other team members and outside agencies, and attending family/review meetings.
