Job overview

The Gastroenterology service at Imperial College NHS Trust is advertising for one substantive full-time post for a Consultant Gastroenterologist with an interest in Hepatobiliary Endoscopy based primarily at Hammersmith Hospital (HH). 

The Gastroenterology service has links to the upper gastrointestinal / HPB surgery at HH and colorectal at CXH. It is linked with Histopathology and Radiology, as well as Dietetics, Bariatrics and hepatology (St Mary’s) teams. The endoscopy units at ICHT provide diagnostic and therapeutic interventional services. All 3 sites are JAG accredited. Services include upper and lower GI endoscopy, bowel cancer screening, capsule endoscopy, ERCP, EUS and stenting; as well as radiofrequency ablation, enteroscopy, cholangioscopy and EUS. 

The Gastroenterology team at Hammersmith Hospital currently provides consultant ward cover on a rotational basis. Plus a consultant-led nutrition round on a weekly basis. The post has no commitment to the Acute Medical Take, but applicants should be confident and competent in the ward management of general medical patients.

At the Hammersmith site there are weekly combined academic medical/surgical/radiology meetings, for benign and malignant HPB disease, liver cancer and a weekly departmental meeting. There are also a weekly cross site MDTs for inflammatory bowel disease, polyps, colorectal / upper GI cancers. Consultants are provided with office space and secretarial support, as well as access to ICT support.

Main duties of the job

The post holder will work closely with other members of the team to ensure high quality acute and general medical and gastroenterology services within the strategic framework agreed by the team.  They will work closely with the appropriate clinical management teams. 

Ward responsibilities, and acute take responsibilities, will be as described in section 2.2 above.

The post holder will undertake 5 – 6 endoscopy lists / outpatient clinics per week.

 

There will be a commitment to the out-of-hours endoscopy service.

Provide High Quality Care to Patients in Acute & Specialist Medicine

 

Ø  The post holder must be medically qualified and maintain GMC specialist registration.

Ø  The post holder must be JAG accredited in EUS and ERCP.

Ø  The post holder must maintain the competencies required to carry out the duties of the post.

Ø  The post holder must ensure prompt attendance at agreed direct clinical care Programmed Activities.

Ø  The post holder must ensure patients are involved in decisions about their care and to respond to their views.

Ø  The Trust supports the requirements of CPD as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities. The job plan includes 1 PA for Continual Professional Development.

Detailed job description and main responsibilities

The post includes consultant ward cover on a 1 in 4 basis. This post has no commitment to the Acute Medical Take, but applicants should be confident and competent in the ward management of general medical patients. There is a luminal consultant who provides support for referrals across the hospital in addition. 

Gastroenterology out-patient services 

·         General Gastroenterology out-patient clinics

·         Inflammatory bowel disease outpatient clinics at St Mary’s, HH and CXH

·         Specialist HPB clinics at Hammersmith

·         Diagnostic / therapeutic endoscopy including OGD, colonoscopy, ERCP & EUS, enteroscopy

·         Capsule endoscopy

·         Breath testing

·         GI physiology

·         Bowel Cancer Screening 

Acute GI Bleed on call 

The on-call consultant covers all three sites for Upper GI Bleeding supported by an SpR rota and an on-call endoscopy nurse. The expected on call intensity will be 1 in 10 and will be remunerated as 1 PA allowance and an on call supplement. Emergency procedures occurring out of hours take place in theatres or in the High Dependency areas with anaesthetics support. Subject to confirmation, the post holder will join this rota.

Qualifications

Essential criteria
  • GMC registered Medical Practitioner
  • Entry on the General Medical Council (GMC) Specialist Register via one of the following: a. Certificate of Completion of Training (CCST) (the proposed CCST date must be within 6 months of the interview) b. Certificate of Eligibility for Specialist Registration (CESR) c. European Community Rights
  • Appropriate Royal College Membership (e.g. MRCP)

Knowledge/Skills

Essential criteria
  • CCST in Gastroenterology

Clinical Expertise in Specialty/Sub-specialty

Essential criteria
  • Independent practice in luminal endoscopy (including therapies) by JAG criteria
  • Independent in ERCP and EUS by JAG criteria
  • Experience managing GI bleed endoscopically
  • IT Skills and computer literacy

Leadership / Management Skills

Essential criteria
  • Able to take responsibility, show leadership, make decisions, exert appropriate authority
  • Have proven skills in leading, motivating, developing and managing the performance of colleagues
  • Management of staff

Teaching & Training

Essential criteria
  • Experience of teaching and training undergraduates / postgraduates and junior medical staff
