Job overview
The Post provides embedded physician support to elective and non-elective general, colorectal surgery on the St Marys site in addition to providing rotational coverage to Albert Ward (surgical step down ward) on the St Mary’s site.
The ICHT Surgical Liaison Service consists of a team of consultant geriatricians who provide embedded medical care to adults of all ages admitted under surgical specialties; major trauma, orthopaedics, vascular surgery, general surgery, neurosurgery and spinal surgery. By effectively addressing medical needs and complexity alongside acute surgical issues they help to provide comprehensive holistic care, prevent complications, optimise outcomes, and give every patient the best chance of making a successful recovery.

Main duties of the job
The Surgical Liaison service also plays a key role in the pre-operative setting, offering a dedicated pre-assessment clinic for patients with frailty or medical complexity before elective operations, known as the Comprehensive Assessment for Surgery (COMPASS) clinic. This clinic helps evaluate and prepare patients for surgery, providing them, their relatives, and the surgical and anaesthetic teams with all the necessary information to make informed decisions about surgery, perioperative care, and post-operative recovery.
Key features of the Surgical Liaison Service include:
- Preoperative assessment and optimisation of frailty, cognitive disorders, and multimorbidity.
- Prognostication and shared decision-making.
- Management of postoperative complications, hospital-acquired deconditioning, and cognitive disorders.
- Rehabilitation, goal setting, discharge planning, and referral to community services.
- Treatment escalation and advance care planning.
- Clear communication with patients and carers throughout the perioperative pathway.
- Collaborative working with other disciplines and specialties across the Trust.

This role is allocated 10 PAs and is primarily based at St Marys Hospital, with a requirement to cover the Albert Ward which is our stepdown ward based at St Mary's Hospital.
The ICHT Surgical Liaison Service is led by a team of consultant geriatricians who deliver integrated medical care to adults of all ages admitted under surgical specialties such as major trauma, orthopaedics, vascular surgery, general surgery, neurosurgery, and spinal surgery.
By addressing medical complexities alongside acute surgical issues, the team provides holistic care that prevents complications, optimises patient outcomes, and supports every patient in achieving the best possible recovery.

Education/ qualifications
Essential criteria
- On GMC’s register with a Licence to Practice or eligible to apply for GMC registration and a Licence to Practice
- On GMC’s Specialist Register or within six months of being admitted to the register by the date of the interviews or eligible for specialist registration
- Appropriate Royal College Membership
Knowledge/skills
Essential criteria
- Subspecialty higher grid completion in surgical liaison or orthogeriatric medicine
- Completion of at least one full year training in a surgical liaison service.
- IT Skills and computer literacy
Leadership/ management skills
Essential criteria
- Able to take responsibility, show leadership, make decisions, exert appropriate authority
- Have proven skills in leading, motivating, developing and managing the performance of colleagues.
