Job overview
The Urology Cancer team currently have an exciting opportunity for a Band 4 Pathway Coordinator position in Prostate Cancer.
Provide a comprehensive and quality administration service to the designated sub speciality of Prostate within the Urological Cancer Team. The Pathway Coordinator will actively manage the administrative processes attached to patient pathways, ensuring a smooth and efficient service for both the multidisciplinary team and patients. The main focus of the role is to provide clinic, and patient management as appropriate.
The pathway coordinator will be responsible for ensuring that cancer care patient pathways are delivered in a timely manner, and that patient pathway records are accurate, up to date, and reflect national and local policies.
Main duties of the job
- To manage any template changes liaising with the Cerner team as well as with medical records staff, main outpatients and the Choose and Book team as appropriate.
- Implement and maintain an efficient internal system and process to ensure the outpatient access target is met for all new referrals to the Imperial College Healthcare NHS Trust.
- Ensure effective communication networks are developed between all organisations that transfer GP referrals to the Urology service.
- Work closely with other administrators and clinic staff regarding individual patient queries to ensure targets are met and patients are treated efficiently and equitably
- Monitor and review patient care and/or treatment progress using the Patient Tracking List in line with Trust and local policies.
- Identify from the Tracking List the next steps in the patient care pathway in line with clinical need.
- Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies.
- Co-ordinate and provide administrative support to the relevant teams to ensure that all the relevant paperwork and clinical information is available

- GCSE level (including English & Maths) or equivalent.
SKILLS/ABILITIES

- Competent use of IT systems including Excel, Word, Outlook.
