Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as “good” or “outstanding” in over three-quarters of the services inspected by the Care Quality Commission.
Every year our work includes performing 10,000 procedures in our theatres; bringing seven and a half thousand new lives into the world; carrying out 570,000 visits to patients in their homes or communities and providing emergency care for more than 300,000 patients arriving in our busy Urgent and Emergency Departments.

Job overview
The Trust is looking to recruit to a substantive Consultant Physician in Geriatric Medicine, with an interest in Community Care, based at the University Hospital Lewisham (UHL) the community teams have been created to improve community-based care, as a safe alternative to hospital admission. The services provide; a rapid comprehensive assessment, supportive management plans and continued monitoring, without having to be admitted to hospital. The Community single point of access covers the Urgent Community Response (UCR) team, the Proactive Ageing Well team (PAWs) and Virtual ward.

Main duties of the job
The post holder would be in addition to the twelve current Consultant Geriatricians, and be part of the wider multidisciplinary specialist Community team. This post has been designed to continue to support and maintain a Consultant led service in the Directorate of Medicine for Older Adults and to develop the well-established Community services.
The post-holder would participate in the Acute Frailty unit on a rotational basis, as well as be part of the overnight GIM rota (currently1:24). The post involves supervision and education of trainees within the hospital.

Detailed job description and main responsibilities
General Responsibilities
· To provide high quality health care for older patients referred for diagnosis and treatment, including outpatients and inpatients.
· To respond to inpatient and outpatient referrals from other specialties
· To provide prompt effective advice for General Practitioners
· To participate in educational and audit meetings
· To meet CPD and revalidation requirements
· To provide cover for consultant colleagues during periods of leave
· To support the Hospital’s Clinical Governance Strategy
· To contribute to all other aspects of the Directorate and Hospital’s affairs, including relevant Consultant and Directorate meetings
Medicine for Older Adults
· To provide a consultant-led service in the specialty
· To attend community multidisciplinary meetings.
· To support and assist in the development of the Urgent Community Response team, PAWs and Virtual Ward
· To contribute to the response of the Directorate to the National Service Framework for Older People
· To contribute to research in Medicine for the Elderly in an area of special interest
· To contribute to meeting the 18 week outpatient target
· To contribute to meeting the 4 hour waiting time target in the Emergency Department
· To contribute to other targets e.g., quality indicators for emergency department

- MBBS or equivalent

- CCT in Geriatric Medicine and General (Internal) Medicine
