Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The Clinical/Care Professional Lead for PHM & Equity is a key post within the Lambeth Together Local Care Partnership, working alongside all Partners including the NHS, Local Authority and our communities & voluntary sector. The postholder will work collaboratively with a number of existing CCPLS, SEL ICS colleagues and other stakeholders including region and national to support the development of a more coordinated and shared approach to reducing health inequalities. In addition to Our Health, Our Lambeth, this will include a focus on the CORE20PLUS5 initiative, which in Lambeth is CORE20PLUS7, due to the addition of diabetes and chronic pain for adults.

• Co-chairing the Lambeth Together EDI Group, which is a sub-group of the Lambeth Together Local Care Partnership Board.
• Working across key teams within our Lambeth Together Partnership to help them ensure the delivery of our Population Health Management & EDI objectives. This includes identifying opportunities for improvement and highlighting concerns or risks via the Lambeth Together Assurance Group, and any other key local fora that have a responsibility for Population Health & Equity. This specifically includes the Council, and the Health & Wellbeing Board, and Primary Care via the Primary Care Commissioning Committee.
• Actively contributing to the Council’s Health & Wellbeing Strategy.
• Providing clinical input for development of and assessment of funding proposals specifically aimed at reducing disparities.
• Acting as a key interface with the SEL ICB PHM & Equity team, supporting Lambeth to provide assurance to, and attending the ICB Population Health & Equity Executive when required, and any task and finish groups that may be set up.

- Qualified care and/or clinical professional i.e., NMC, HCPC, social work register, GMC, etc.
- Postgraduate degree or higher professional qualification
- Evidence of continuing professional development
- Project Management or programme management certification or equivalent experience.

- Understand the challenges of health inequalities and principles of identifying and addressing these
- High level analytical skills and the ability to draw qualitative and quantitative data from a wide range of sources and present in a clear concise manner.
