Urgent & Emergency Care Flow Initiatives Clinical Lead (UEC)

CCPL Tier 2

Job overview

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The UEC Transformation Programme covers a number of key work areas with the aim of ensuring an equitable offer across SEL with patients receiving safe care in the most appropriate environment without unnecessary delay. For patients who require a hospital admission we work to ensure they do not remain in a hospital bed for longer than necessary, but rather receive a safe and timely transfer of care or discharge home.

The Flow Improvement Clinical Lead for SEL is expected to work with multiple services, stakeholders and providers across the SEL footprint as well as represent the ICS at London-wide discussions. The post holder will work collaboratively with the ICB Urgent & Emergency Care team, as well as other clinical leads supporting the urgent and emergency care programmes of work.

Provisionally Wednesday 5th June pm (TBC)

Main duties of the job

The Flow Improvement Clinical Lead for SEL will provide clinical leadership to SEL ICS through a number of key responsibilities, including:

•    Provision of specialist knowledge and experience of improvement implementation related to UEC flow, particularly in relation to patient transfer of care and discharge.
•    Building collaborative working relationships with system partners that play a key part in transfer of care, for example, local authority leads, acute and mental health ward leads, social workers and community services.
•    Galvanising system engagement in and commitment to relevant SEL programmes of work related to flow, for example, reducing long length of stay, improving weekend discharge and embedding of criteria led discharge.
•    Clinical leadership of projects related to UEC flow across SEL including local delivery of national initiatives being implemented by the ICB.
•    Supporting the delivery of improvement projects related to flow as required
