Job overview
The post holder will be required to support the delivery of care currently in the form of New Models of Care, enabling the service to meet the London Wide service specification standards for Prison Healthcare.
The post holder will assist in providing a high quality, high performing mental health service for service users within HMP Wormwood Scrubs who are presenting with mental health problems in line with the New Models of Care.
The post holder will work alongside clinical & discipline staff to provide individualised assessment, care/management plans and risk assessment for a small caseload of often challenging patients, preparing them for release and then providing short-term support through the gate within the community. You will be a confident practitioner that thrives in working in a fast-paced often high pressured environment.
The successful candidate will assess the service users, track
them through their sentence, ensuring that there are
comprehensive release plans in place, and meet them upon release to
support them through the first 24 hours following release, assertively linking them into treatment and support resources.
A holistic release plan will be drawn up with the service user in conjunction with Transfer and Release team in order that all necessary referrals can be made prior to release, and care pathways established
for each individual service user.
Main duties of the job
Liaise with medical services, where appropriate, to ensure service users have access to any necessary medical care upon release.
Undertake assessments of service users in prison, to co-facilitate pre-release plans in order to develop comprehensive release plan plans for our service users.
To develop links with other relevant agencies that offer aftercare support across England ensuring service users have access to appropriate support post release regardless of their area of release.
Be responsible and accountable for the management of small caseload as directed by the Team Leader
To actively support the CPA process.
Jointly facilitate group work with multi-disciplinary colleagues where appropriate
To complete detailed clinical risk assessments at first and subsequent assessment interview, and accurately record and report key risks
To develop care plans in collaboration with service users, carer and other relevant person or agency
To be conversant with all guidance and operational procedures in relation to offender healthcare at HMP Wormwood Scrubs.

Person specification
Education and Qualifications
Essential criteria
- Minimum of Health and Social care NVQ Level 3
Desirable criteria
- Degree level or above in Psychology, Counselling or relevant field.
Experience and Knowledge
Essential criteria
- Experience of working within a secure/forensic or prison setting
- Experience of working with challenging and complex patients
- Either currently working as a Band 4 or experience of working in a Health or Social Care setting
Desirable criteria
- Experience of lone working
Skills and Abilities
Essential criteria
- The ability to work in often difficult circumstances within a high pressured environment
- Experience of carrying out complex assessments including risk
