Job overview

Applications are invited for the position of Senior Clinical Fellow (previously known as Trust Registrar) in Acute Medicine, at the North Middlesex University Hospital, Royal Free London NHS Foundation Trust in North Central London (NCL).

This is an exciting time to join the department as we recently joined the Royal Free group and have the opportunity to realign our services to the needs of our patients.

The successful applicant will work in the Same Day Emergency Care Unit (SDEC), providing medical care in an ambulatory setting to patients presenting with a variety of medical conditions referred via multiple pathways (general practice, London ambulance service, emergency department, NHS 111, acute medicine department).

The SDEC team is currently expanding to accommodate increased activity throughout the week, in line with the expansion of admission avoidance pathways. We have both condition-specific pathways (for high volume conditions, such as DVT, cellulitis, pyelonephritis, etc) and inclusion and exclusion criteria, allowing us to maximise the number of patients seen in SDEC thus reducing the number of hospital admissions and the number of patients seen in the emergency department.

Main duties of the job

MAIN DUTIES AND RESPONSIBILITIES

1.    CLINICAL RESPONSIBILITIES

1.1 Keeps professional knowledge & skills up to date, follows infection control standards.

1.2 Able to discuss treatment options with patients in a way they can understand.

1.3 Able to prioritise according to clinical need.

1.4 Competent in bedside medical procedures, physical examination, history taking.

1.5 Competent in formulating investigation and treatment plans, managing complex scenarios and deteriorating patients, leading cardiac arrest teams.

1.6 Provides senior medical support and supervision to resident doctors and junior clinical fellows, including assessment, management, and review of acutely unwell patients.

1.7 Provides timely and appropriate escalation to consultants for complex or deteriorating patients.

ABOUT THE HOSPITAL:

North Middlesex Hospital, Royal Free London NHS Foundation Trust is a medium-sized District General Hospital based in Edmonton in Enfield, North London. We serve a diverse multicultural population largely from the London boroughs of Enfield and Haringey. Many of our patients live in wards that are within the 5% most deprived in the UK and a large proportion of our patients were born abroad. This makes it both a fascinating and a challenging hospital to work in. 

Over the last decade we have rebuilt most of the hospital and now have facilities that we are proud to work in. We employ over 3,500 staff, more than half of whom live locally. We encourage apprenticeships from the local community and work closely with our local Health Watch. 

We are primarily an acute hospital with more than 90% of our bed days being used for patients admitted via our emergency and SDEC units. As well as offering everything you would expect from a major acute hospital, we have a number of tertiary services treating patients with HIV and Sickle Cell Disease and a large cancer and radiotherapy service. We also run our local community Sexual Health Clinics. 

We are proud of our staff and want to ensure their training allows them to provide excellent clinical care. We are also a training unit for medical students from UCL and St George’s University Grenada, and for nursing and midwifery students from Middlesex and City Universities.

Essential criteria
  • Eligible for full GMC registration and licence to practice
  • Postgraduate qualification (e.g. MRCP or equivalent)
  • MBBS or equivalent

Experience

Essential criteria
  • Competent in bedside medical procedures, physical examination, history taking, formulating investigation and management plans
  • Provides senior medical support and supervision, including assessment and management of acutely unwell patients
Desirable criteria
  • Knowledge of current issues in the NHS
