Job overview

This role is an exciting opportunity for an enthusiastic candidate who wishes to work with the adult patient group in a friendly and motivated multi-disciplinary team at Canterbury/ Capetown wards based at Chase farm hospital , Enfield.

This service provides short term Inpatient nursing and therapy services to patients admitted for rehabilitation, reablement and recovery promoting independence and encouraging integration back to the community. The post holder will provide therapy assessments and interventions within the remit of their role to inpatients under the supervision of the senior therapy clinicians within the team. 

The post holder will be required to support the delivery of rehabilitation and recovery programmes including individualised and group based therapeutic sessions and education / enabling programmes and to visit patients in their own homes to carry out prescribed treatment and assess for and provide appropriate mobility aids and equipment directly or by prescription. 

The postholder may be required to work at various sites as part of the role to support Inpatient services, including out in the community in people’s homes and be part of a 7 day therapy service as directed by the Integrated Therapies Lead. 

Development is encouraged through regular supervision and CPD opportunities and the post holder will work closely with colleagues within the wider therapy team.  

Main duties of the job

CLINICAL RESPONSIBILITIES

To undertake clinical work delegated by the qualified therapy staff including Physiotherapist & Occupational therapist and at times other AHPs.

To be responsible for a designated caseload of patients. 

To work effectively as a member of a team in providing a programme of ward based therapeutic activities - individual & group based. 

 To contribute to all stages of the therapeutic process including:  

 Information gathering through liaison with clients, their informal carers, and colleagues from health and other agencies. 

Completing simple assessments, through interview and observation of functional tasks. 

Provision of aids and equipment.  

 Identifying needs and forming goals and plans for interventions. 

Completing a wide range of interventions including: - giving therapeutic advice; mobility, transfers, functional, ADL based rehabilitation programmes, enabling self-management of conditions through patient and carer education, home assessments and home-based therapy programmes, equipment assessment, prescription and safe use within the ward based as well as home environment. Evaluating patient progress through regular re-assessment and ensuring continuous progression by graded activity through a 1:1 or groups-based approach. Contribute to formal therapy assessments and provide feedback. 

Review of patients after completion of minor adaptations to home environment.

Working for our organisation

Canterbury ward ( 17 beds) , Capetown ward ( 38 beds) and Magnolia unit (34 beds) are the 3 Inpatient rehabilitation wards   within the trust based on the Chase farm hospital & St Michael's sites . These services provide intensive inpatient rehabilitation and recovery therapy programmes to adults admitted for a short stay.

Discharge planning including completion of appropriate referrals and reports, home visits and follow up visits as directed by the qualified therapy staff. To use clinical reasoning within your role remit throughout the therapeutic process providing a rationale for your assessment, interventions and changes in patient presentation.

Person specification

EDUCATION AND QUALIFICATIONS

Essential criteria
  • Relevant qualification e.g. Health & Social Care NVQ 3 or equivalent.
  • Training courses relevant to care group Physiotherapy, Occupational therapy, Inpatient, ward based.
  • English qualification at GSCE or equivalent level.
Desirable criteria
  • Speech and Language Therapy related training.

EXPERIENCE AND KNOWLEDGE

Essential criteria
  • Experience working within a rehabilitation setting, delivering care to patients with physical or mental problems.
  • Experience working within a rehabilitation setting, delivering care to patients with physical or mental problems. Evidence of working with and caring for adults either in an acute / intermediate care wards or community NHS setting.
  • Understanding of therapy outcome measures and assessment tools.
  • Knowledge of the ranges of equipment relevant to clinical area.
Desirable criteria
  • Use of Careflow/ EPR/ RIO

SKILLS AND ABILITIES

  • Ability to work autonomously & set own priorities
  • Therapy rehabilitation skills- Physiotherapy, Occupational therapy
