Job overview
A fantastic opportunity has arisen to join the Cardiology Team at Royal Free London Trust in managing the cardiology virtual wards. Our ambition is to develop a virtual ward model that is both ‘step up’ from primary and community care to safely avoid hospital admission* and ‘step down’ from the hospital. This role would lead on the management of patients using this platform and the governance supporting their care. The post-holder will be required to work autonomously in delivering independent assessment for the patients on virtual wards
The post-holder will be expected to work within the four clinical pillars of advanced practice, clinical practice, facilitating learning, leadership and evidence, research and development. This will be demonstrated within their portfolio
The post-holder will be required to work autonomously in delivering independent assessment for the patients on virtual ward. The role will be structured around four functions, delivering expert practice, professional leadership/consultancy, education, training and development, service development, research and evaluation. .
The post is working cross site at Barnet and Hampstead site.
Main duties of the job
· To act as a high-level autonomous practitioner and role model, delivering advanced practice and promoting clinical effectiveness and patient centered care.
· The post-holder will be required to work autonomously in delivering independent assessment for the patients on virtual ward.
· The post holder will demonstrate a high level of advanced expertise, specialist knowledge, demonstrate high levels of judgement, discretion and decision making in the delivery of clinical care.
· The post holder will also provide expert advice delivering advanced clinical practice, clinical leadership and supervision, support and mentorship for multidisciplinary professionals including other ACPs, junior medical staff and clinical nursing staff of all grades. They will also be responsible for enhancing the quality of the service by providing education and support to all practitioners and acting as a role model.

Essential criteria
- MSc Advanced Clinical Practice or equivalent qualification
- Evidence of post registration qualification relevant to specialty
- Mentorship/ teaching and assessing qualification
- Independent and supplementary prescribing
- Registration as a Nurse/Midwife/AHP
- First level healthcare degree
Desirable criteria
- Leadership qualification
Experience

- Previous experience at senior nursing level Band 7 or above
- Evidence of ability to work & lead autonomously at advanced practice level
- Evidence in challenging clinical practice and leading in the development of new pathways of care and delivering best practice
- Evidence of successfully resolving complex problems.
- Experience in carrying out clinical audit and risk assessment
- Proven skills in leading, managing and developing teams
