Respiratory Clinical Nurse Navigator

An opportunity has arisen for a full time Band 6 Nurse Navigator to join the team at UCLH. This is for 6-7 months to cover maternity leave.
We are looking for an enthusiastic and innovative team member, with some respiratory experience. The role will require excellent communication skills. Please contact Lead Nurse for information in relation to the role. We would consider a secondment for internal applicants.

The role will include;
• Supporting patients through the National |optimal Lung Cancer Pathway (NOLCP) and lung screening programme
• Telephone pre-assessment service to ensure all patients are fir for procedure and to give guidance
• Deliver patient centred care by acting as a point of contact and keyworker for patients and GP’s ensuring there is a community link with primary care
• Support the Respiratory clinicians in the efficient co-ordination and organisation of diagnostic tests providing an important link between theatres, anaesthetics, and the Respiratory department
• Provide advice and refer on or signpost to other sources of support for patients
• Deliver a weekly nurse-led telephone results clinic to patients
• Participate in and support research and audit programmes/objectives for the Respiratory department
• Participate in a programme of self-development, clinical supervision and individual performance review (IPR)
