Discharge Support Workers

West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.
We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

The discharge hub at West Middlesex Hospital was set up initially as a response to the COVID-19 pandemic. The discharge hub brings together West Middlesex Hospital, Hounslow and Richmond Community Healthcare and Adult Social Care to ensure patients are discharged in a safe and timely manner
Main duties of the job
We are looking for individuals who enjoy working as part of a team in a complex and fast changing environment. You will be responsible for supporting the clinicians in the discharge hub to ensure patients are transferred in a safe and timely manner. You need to be a great communicator who pays attention to detail and works in a methodical way.

- Educated to A Level/ NVQ Leve 3 or equivalent
- Higher National Certificate in health and social care or equivalent
Desirable criteria
- ECDL qualification
- Knowledge of medical terminology

- Foundation degree level or equivalent
Experience
Essential criteria
- Relevant and recent experience working in a senior Administrative role
- Experience of working in a hospital health care setting
- Experience of using NHS IT/ Database systems
- Experience of discharge processes
- Working knowledge of office procedures, structure and workings of a large organisation
