Job overview

NHS reforms and government directives have placed primary care in the forefront of health care provision. District nursing is undergoing a huge transformation with a multitude of new roles, responsibilities and career structures.  

The senior professional development nurse will work alongside the lead district nurse for professional development, district nurse team managers and deputy team managers to enable nursing associates in the London Boroughs of Islington and Haringey to develop their clinical, interpersonal and communication skills, plus professional attitudes. The post holder will support the lead district nurse to ensure that regular service audits take place.

The post holder will assess and supervise trainee nursing associates in clinical practice and provide one to one supervision and teaching until the required level of competence and skills are achieved. 

The post holder will be required to work across both Haringey and Islington and will be required to work occasional twilight and well as day shifts.

Main duties of the job

Managing Care

•    To have responsibility for the effective and efficient delivery of nursing care to patients as delegated by District Nurse Team Leader.

•    Work one to one clinically with the TNAs providing feedback to the individual on their performance as well as to their Practice Assessors/supervisors and team managers.

•    To undertake skilled nursing activities directly related to the assessment, planning and evaluation of patient care and the supervision and training trainee nurse associates.

•    To promote personal, professional, and clinical expertise through regular evaluation and updating of practice.

•    Work with the team managers to identify new practice assessors and supervisors and support them through the PA/PS programme.

•    Work with Academic providers, programme leaders and university link lecturers to ensure good communication and achievement of all formal assessment priorities.
•    Act as a role model and peer support in the clinical environment.

•    To fulfil requirements of the Community Care Act, through effective liaison with Social Service Departments, Local Authorities and providers of acute care plus other agencies.

•    To undertake health promotion, including chronic disease management, for patients and their families on the current caseload.

•    To be involved in care/case management under the supervision of the District Nurse Team Leader, including of clients with continuing care needs.

Detailed job description and main responsibilities

Managing Care

Managing Resources

  • To teach and supervise the clinical practice of trainee nurse associates and to appraise and to identify training needs. In conjunction with the District Nurse Team Leader, meet the training needs of the TNAs.
  • To promote good communication, employment practices and industrial relations within the nursing team.
  • To lead team (and other) meetings where appropriate.
  • To ensure the successful implementation of appropriate organisational arrangements within the team, so that the overall aims and objectives of the service are achieved.
  • To manage resources to ensure they relate to the overall objectives and responsibilities of the team.
  • To comply with financial standing instructions.
  • To be able to use the computer to: write written reports in Word, access/send emails on the Whittington Health system, access the intranet/internet for relevant information according to Whittington Health policy.
  • To be prepared to undertake training in using Protocol, the Whittington Health/SS joint single assessment process computer held document.

Record Keeping

  • To ensure accurate, contemporaneous records are kept that ensures safety and continuity of patient care (including electronic records where appropriate).
  • To ensure accurate statistical data is promptly submitted in line with Whittington Health Policies.

Quality

  • To participate in policy and standard setting, quality initiatives and clinical audit.
  • To be fully aware of the clinical governance agenda.
  • To ensure that the care given meets the standards and policies set by Whittington Health. To inform line management of constraints that will prevent these standards from being met.
  • To respond promptly to any incident where service quality has fallen below an acceptable standard and report to the District Nurse.
  • To attend regular clinical supervision and promote and support team attendance as per Whittington Health policy
  • To appraise junior members of the team using the Knowledge and Skills Framework
  • Whittington Health is an integrated care organisation and there may be occasions where staff are required to rotate between clinical environments to meet service need. This could be on a long or short term basis.

 Professional development and responsibilities

  • Where the post holder has particular expertise, act as advisor/resource person to other staff.
  • To participate in training as appropriate of trainee nursing associates.
  • To participate in resource projects and health promotion programmes as requested.
  • To undertake duties as delegated by Lead Nurse.

Research

  • To participate in agreed research projects and/or innovations subject to discussion with the District Nurse and following agreed policy and protocol.
  • To contribute to the development of the nursing team and the community health services by participating in working parties, committees and planning and discussion groups

Education and Qualification

Essential criteria
  • Relevant BSc/ Diploma in Adult Nursing or equivalent
  • Registered with the Nursing and Midwifery Council
Desirable criteria
  • Practice Assessor

Skills & Abilities

Essential criteria
  • Confidence to engage in appropriate dialogue with other clinicians and professions around complex clinical cases.
  • A good understanding of integrated community services
  • Good written and oral communication skills and able to communicate with patients and all grades of healthcare and social care staff (verbally and in writing).
  • Good negotiation skills and the ability to influence senior district nursing team, the multidisciplinary team and management.
  • Experience of supervising and training junior staff and wider district nursing team members.
  • Experience of providing education and training to a range of professionals including, Nursing and social care
  • Good interpersonal and negotiation skills.
