Job overview
The post holder will work in the Islington Rapid Response and Virtual Ward (RRVW) team which is part of the wider multi-agency Islington Urgent Care Response service which supports hospital discharges and admission avoidance.
RRVW is co-located and integrated with therapy and adult social services colleagues and aims to respond to patient referrals in the community within 2 hours to prevent admission to hospital.
The post holder will primarily be working in Islington borough but may be required to see patients in other NCL boroughs.
Main duties of the job
- Participate in the assessment of care needs and the implementation of programmes of care in partnership with patients/carers in their own homes
- Identify changes in patients conditions and, within their capabilities, recognise where significant changes in the care plan are required and report these back to the senior nurse on duty in a prompt and timely manner.
- Liaise effectively with other nursing and multi professional team members within the scope of the health care Assistant role and report back to the nurse in charge.
- Provide administrative support to the team.

Detailed job description and main responsibilities
- Demonstrate a high standard of care within Whittington Health and national quality assurance frameworks.
- Always adhere to Whittington Health/UCLH policies and procedures.
- Organise own workload and establish priorities (and that of other unregistered nurses where appropriate). In collaboration with line manager/nurse in charge
- Liaise effectively with members the wider multidisciplinary team within the scope of the Health care Assistant role, reporting back to the nurse in charge as appropriate.
- Initiate onwards referrals to other agencies, including Social Services, where appropriate
- Participate in the orientation of new staff.

- NVQ Level 3 in a related subject or equivalent experience
