Job overview
To provide high quality effective physiotherapy assessment and short-term treatment/management programs for adults presenting a range of conditions in Haringey who require a community based rehabilitation service.
To use specialist physiotherapy knowledge and skills to manage own complex caseload, as an autonomous practitioner.
To act as a source of expertise on Falls, orthopaedic and community rehabilitation liaising closely with specialist services; providing advice to patients, carers, all grades of physiotherapists, other health care professionals, consultants, general practitioners and social services.
To support the use of evidence-based practice within the service, incorporating national guidelines e.g. falls guidelines into practice.
To assist with the running of strength and balance classes in the community.
Main duties of the job

To deliver patient care based on principles of best practice, achievable through direct patient contact or via supervision. To carry out specialist, comprehensive assessments of patients with a complex presentation, using investigative and analytical skills formulating individualized management and treatment plans.
To provide specialized physiotherapy interventions at home or in a community setting as required, collaborating with colleagues in the community and secondary care health professionals to promote continuity of care.
To ensure a timely response to hospital discharges in line with Haringey’s Trust policies.
To provide specialist assessment for and provision of multi-disciplinary equipment in line with national and local guidelines.
To undertake joint visits with all multi-disciplinary team members where appropriate and provide specialist advice on treatment/ management regimes.
To keep accurate electronic treatment records, which fulfil legal and statistical requirements, and which are in accordance with HPC, CSP, Trust and ICTT documentation standards.

Detailed job description and main responsibilities
To work with patients and their carers when appropriate in such a way as to promote their self-management strategies.
To represent the service at the multi-disciplinary team meetings. This will include discussion of patient care, progress and involvement in discharge planning and continued rehabilitation needs.
To keep accurate treatment records, which fulfil legal and statistical requirements, and which are in accordance with HPC/CSP/Trust/departmental standards.
To provide comprehensive progress and discharge reports to GP’s, consultants, patients, members of the multi-disciplinary team and other patient services.
To manage clinical risk within own patient caseload at all times.
To work flexibly to provide cover in other clinical areas when required due to staff shortages.

Qualifications
Essential criteria
- Holds a recognized degree or diploma in Physiotherapy
- Registration with the Health Professions Council
Desirable criteria
- Attendance on courses related to Physiotherapy e.g. Falls prevention
- Member of the CSP and relevant Clinical Interest group e.g. AGILE
Skills & Abilities
Essential criteria
- Essential IT skills, including Word and PowerPoint.
- Able to communicate complex and sensitive information in written, verbal and presentational form to patients, carers and staff within the Trust and across other organisations.
- Able to apply problem-solving skills to complex situations, which may include clinical or organisational issues.
Experience
Essential criteria
- At least 2 years post-registration experience, including rotational
- experience and evidence of dedicated clinical practice and post-graduate
- professional development in relevant areas of physiotherapy
- Experience of working in the NHS
Knowledge
Essential criteria
- Knowledge of current NHS priorities and policy
- Understanding of clinical governance activity and its application to physiotherapyy services and practice.
- Knowledge of Physiotherapy professional issues.
- Knowledge of other medical conditions which you may encounter including, but not exclusively, respiratory, orthopaedics and conditions of old age.
Desirable criteria
- Evidence of attendance of post-graduate training related to the post
- knowledge of the audit cycle and its uses.
Experience
Essential criteria
- Experience of running Falls/Balance/other groups
- Experience of organising, writing and delivering in-service training programmes.
- Experience of involvement in clinical governance/service improvement initiatives
Desirable criteria
- Experience of taking physiotherapy students on clinical practice placements.
- Experience of working with the voluntary sector
- Experience of supervising junior and non-qualified staff, including participation in personal development reviews.
Skills & Abilities

- Able to apply problem-solving skills to complex situations, which may include clinical or organisational issues.
- Well-developed planning skills.
- Ability and competency in using a range of therapeutic skills including but not exclusively, various assessment techniques, handling skills, manual handling techniques, exercise therapy, and splinting.

- Commitment to holistic patient management and the promotion and progression of physiotherapy in the community.
Knowledge
Essential criteria
- Up to date and advanced knowledge of a variety of physiotherapy concepts and approaches, and of current best practice within stroke rehabilitation.
- Knowledge of Neurological condition pathologies including stroke and their management.
- Knowledge of the falls strategy, and the NSF for long term conditions and related frameworks.
- Knowledge of outcome measure tools relevant to community Physiotherapy and rehabilitation, and ability to select and justify their use.
- Demonstrate an understanding of responsibilities under the Health and Safety at Work Act and of assessing risk within clinical work.

- Developed clinical reasoning skills with an ability to develop client centred, realistic and achievable goals.
- Able to assimilate and analyse complex clinical information.
