Community Health Services Transformation and Digital Delivery Lead

NHS AfC: Band 8a

ELFT has long been recognised as a centre of excellence for mental and health care, innovation and improvement. So it is a very exciting time for you to come and work for us. Our mission is to make a positive difference to people's lives by improving quality of life for all we serve. Our values are: We Care, We Respect, We are Inclusive – so we are looking for people who live and breathe these qualities when supporting service users and carers, and in their relationships with colleagues in the Trust and our partner organisations.

Job overview

Are you an experienced change leader who excels at the convergence of clinical care, digital innovation, and operational transformation?

As the CHS Transformation & Digital Delivery Lead, you will be the crucial strategic link between frontline clinical teams and the central Trust Digital Directorate. Your role will be to lead, shape, and deliver complex, high-impact transformation and digital enablement projects from their initial stages to establishing them as Business As Usual (BAU).

You will take the reins in managing multidisciplinary project teams, transformation analysts, and seconded clinical staff. Your responsibilities will include overseeing business process redesign, mapping clinical workflows, enhancing electronic patient records (EPR), and ensuring compliance with clinical safety governance.

We are looking for an ambitious, energetic, and adaptable leader who

• Holds a Master’s degree or possesses equivalent high-level work experience, complemented by a recognized project management qualification (e.g., PRINCE2 or MSP).
• Has a proven history of successfully delivering complex digital or operational transformation projects within health or social care contexts.
• Excels at breaking down complex technical and digital concepts into straightforward, business-friendly language
• Demonstrates exceptional negotiation and change management skills, guiding clinical teams through the process of service redesign.
• Shows a strong commitment to Quality Improvement (QI)

Main duties of the job

The CHS TRANSFORMATION & DIGITAL DELIVERY LEAD is a key member of the Community Healthcare Services (CHS) leadership and transformation team. Working cross-divisionally across both the East London and Bedfordshire footprints, this role leads, manages, and delivers complex service transformation and digital enablement projects across the CHS directorate from inception through to business as usual (BAU).

The post holder serves as the primary bridge and interface between CHS clinical/operational services and the central Trust Digital Directorate. The role is responsible for identifying, scoping, and translating clinical service transformation needs into clear digital requirements, business cases, and operational delivery plans. When digital initiatives require operational preparation, pathway redesign, data alignment, or workforce change within CHS, this role holds primary responsibility for managing and executing those service-side workstreams.

The role is responsible for matrix management of an identified project team responsible for the definition and delivery of transformation and digital projects within CHS.

Working for our organisation

Originally formed in 2000, ELFT has long been recognised as a centre of excellence for mental health care, innovation and improvement. We care for a population of more than 750,000 and we are proud to serve one of the most culturally diverse parts of the UK. In Bedfordshire and Luton, we are now responsible for a further 630,000 people’s mental health and community health care needs. Wherever patients receive our services and from whichever team, our values of care, respect and inclusivity underpin all that we do. Valuing the diversity of our local population is fundamental to realising our vision to provide outstanding healthcare to our diverse community. In September 2016, the Trust was proud to be rated as ‘Outstanding’ by the Care Quality Commission, and the Trust was once again rated at ‘Outstanding’ by the CQC in June 2018 and 2021.

We offer:

  • Flexible Working: Flexible and hybrid working arrangements supporting a healthy work-life balance.
  • Professional Growth: Generous continuous personal development (CPD) support, leadership development programmes, and active Quality Improvement (QI) coaching.
  • Benefits: Access to the NHS Pension Scheme, generous annual leave entitlement starting at 27 days plus bank holidays, national NHS staff discounts, and comprehensive health and wellbeing support.
